Does this mean you did not get my message suggesting you take enough glucose carbs to get your bedtime bs above 7.0 before dosing Lantus? I would like to avoid your having a rebound effect, if that is what is causing your high fasting bs in the morning. Taking a couple of glucose tablets will raise your bs above 7.0, and this might help avoid those rebound effects of extremely high fasting blood sugars in the morning. However if you do not take a couple of glucose tablets, and your fasting bs in the morning is high, this would definitely indicate a need to reduce your Lantus dose, if you take Lantus when your bs is in the very low part of the therapeutic range of 3.9-6.66. I would feel better about dosing Lantus if your bs was nearer the top part of the range. I believe it would be wiser to dose Lantus at night with a bs reading nearer the 6.66 rather than the 3.9. Dosing Lantus on a bs of 4.2, in my opinion, is not safe. In other words you did not take any carbs after you reported a bs of 4.2, and you also took the 8 units of Lantus. Do you understand the rebound effect? Guest guest Posted May 8, 2005 Report Share Posted May 8, 2005 Tonight you have a bs of 6.1 which is getting closer to the top of the therapeutic range. Still I would recommend taking one glucose tablet to push it up a little higher. If you wake up in the morning with a high fasting bs, and you do not take this glucose tablet tonight, then I would think you may be having a rebound effect. However if your bs is in the normal range in the morning, I would suggest always keeping your bs at the upper level of normal or even slightly higher when dosing Lantus at night. I know they say that Lantus has an even effect on the bs, but for me I have always noticed a 20-40 USA point drop in my bs from the reading at bedtime and the fasting reading the next morning. This would be approximately a 2 point mole drop equivalent caused by the Lantus. Of course everybody is different, and I don't know how it affects you. If your bs is at 4.2 at bedtime and you dose Lantus, this could drop your bs during the night to a 2.2 level, which can definitely trigger a rebound effect in some people. The number to convert from one reading in USA or British moles is 18. Either divide or multiply by this conversion number of 18 and you will know what the reading is in these countries. For example to convert your USA reading of 128 into moles, just divide it by 18 and you get a mole reading of 7.1. While those insulin mixtures are very useful and considerably less expensive than a single long-acting insulin or a single short-acting insulin, I would recommend getting onboth a long acting insulin like Lantus and a short acting insulin like Humalog or Novolog. By using both of these insulins separately instead of those mixtures of insulin, you can master better and tighter glucose level control. With mastery of glucose level control comes fewer diabetic complications. Guest guest Posted May 8, 2005 Report Share Posted May 8, 2005 for a period of time I was having hypo's in the evening even though I had eaten a cup of rice and fruit, I was only having 4 units of Humalog. The only problem I found with having these hypos in the evening was that I would wake up with my readings above 10. So I decided to cut back on my Humalog to 3 and haven't had any more night time hypos and my morning level is usually below 8. I also should mention that due to a drop in my night level was due to the extra walking I had taken up. Instead of only doing 50 minutes walking a day I'm now doing over an hour. This is spread out between morning and afternoon. My daily insulin dosage consist of 20 units of Lantus in the morning. Humalog - 5 morning, lunch 4, tea 3. These will fluctuate depending what I've been doing during the day and what my levels are at each meal. Another suggestion is maybe have something with some carbs for your evening snack to get you through the night. Such as a glass of milk, a slice of fruit loaf, a slice of multi-grain bread or a piece of fruit. In addition you mentioned about that you should write a book in a previous e-mail. well, why not! I've published a book and it's called " 's Roller-coaster Ride A journal of living with type 1 diabetes. " Of course it helped with me having written journals since my diagnosis or otherwise I would have not remember how I truly felt in my first 10 years of diagnoses. So you should get those fingers moving over the keyboard and see what you can turn out!!
