Hola:

I know that there are a couple of companies such as HEARTFAX that

provide migrant populations with the ability to have their medical

records transferred anywhere they are being seen for a clinical visit.

I can't seem to find the information for them or even a phone number.

Can anyone help me?

: Heart Fax "uses state-of-the-art technology to store and transmit by facsimile communication vital patient medical information to any hospital, emergency facility, or physician anywhere in the world". Their web address is www.heartfax.com, but I got a request for a password on trying to enter it.

Another option for records transfer and tracking of mobile patients is the Migrant Clinicians Network's Health Network--see www.migrantclinician.org/network. The Health Network allows health centers to enroll patients for records transfer and care coordination who are under care for tuberculosis, diabetes, cancer screening and pregnancy care. Detailed information and forms are located on the website. For staff training by telephone or on site, call 512-327-2017.

In California, an organization called MiVIA provides such a service.

They might know of others doing similar work.
