Their web address is www.heartfax.com, but I got a request for a password on trying to enter it. Another option for records transfer and tracking of mobile patients is the Migrant Clinicians Network's Health Network--see www.migrantclinician.org/network. The Health Network allows health centers to enroll patients for records transfer and care coordination who are under care for tuberculosis, diabetes, cancer screening and pregnancy care.
