Guest guest Posted January 5, 2006 Report Share Posted January 5, 2006 A regular pediatrician can diagnose apraxia, and write a letter of medical need/necessity for you. However it " means " more coming from a developmental pediatrician or a pediatric neurologist (which is interesting because the ped neurologists I know, admittedly know very little about apraxia and will tell you they are not the best for the diagnosis - so it depends on the experience of your specialists). The person most trained to make the diagnosis is the SLP. I've been told by our regional center that the report from the SLP has the most weight, but helps to be " supported " by an MD - though not absolutely necessary. I thought it was too risky to go in with just the SLP report without a supporting diagnosis when asking for more services. I also wanted someone else who knew what they were doing to tell me if this really was apraxia or not. Sounds like your SLP is waffling. She should be able to make the call. Ask her to do a Kaufman apraxis test. My 2 year old was able to do it - but the child does need to have sufficient concentration to get through it. We broke up the test over 3 sessions. But also key to have the right diagnosis. You could also get an evaluation from a PROMPT SLP for apraxia - a group well versed in apraxia - to make sure you are going in the right direction. - [ ] Re: Help Hello All! I am facing my Annual review meeting for my son next week- Have actively been searching for a PROMPT trained therapist, and have found two! at the children's Hospital located only a few blocks away from me (I still cannot believe my luck!) My question is- right now even tho he turns three in a couple weeks, we are able to keep him under EI until August of 2006. At that point, CPSE (from their recent evaluation of him) will only be recommending speech therapy three times a week. Currently under EI he receives speech 2X, OT 2X and SpecEd 2X/ week. My Service coordinator just told me he can receive up to five sessions of speech therapy- if he has a diagnosis. His current speech therapist suggests that he has apraxia, but says she doesnt know for sure. I feel she may be trying to hang on to us for as long as she can as a client, as she wont be able to provide therapy for him when he turns three (altho, maybe she could continue thru august now that we will continue under EI). In other words, she knows if he has a diagnosis of apraxia i will change therapists and get a PROMPT trained therapist- which she is not. This is the feeling i hate to admit ive been feeling for a couple months now, and now i am mad at myself for not trying to figure this out back then. In any case, and sorry for the rambling, but this leads me to my two important questions A- who needs to diagnose him as apraxic? Can his regular pediatrician do it? Id like to have the formal diagnosis for the meeting next week, so i can have them up the therapy to five times a week. And Also, i see here and there posts about not being able to diagnose until they're three years old, and then i see posts where they were diagnosed at first meeting with a speech therapist, at a much younger age. Are you active in a local church? If so go to them and ask them to take up a collection for your family. There have been members who have auctioned off advertising space on the bands on Ebay. You can go to local businesses and try and get sponsors, tell them you will put the name of thier business on the bands if necessary. Contact the local papers and give them the story. Get the stars and stripes to do an artcle and maybe you can shame them into action. Contact relatives and see who can send what to help. I know this is not the way it should be, but maybe you can get them started in bands while you continue to fight Tri Care. It appalls me that our country treats our veterans this way, I live this on a daily basis as my father is a vet of WWII and Korea and has been involved with the VFW and Purple Heart. How old was he when he graduated? You are doing the right thing by calling and they will be able to help you. Did you have an exit cast done when he graduated? They can compare to an exit cast, or take new pictures and measurements to compare against exit photos and measurements. Sometimes it is just the way the head is growing at that time that makes any leftover flatness look worse. I just went through similar fears, so I know how you feel. Let us know what happens. So Cranial Tech didn't tell you if they thought she needed to be banded or not?? I know that it may seem kind of scary at the thought of your daughter being banded, but it's not as bad as you may think it is. I felt the same way you did when I found out my daughter had to wear a band, I cried because I was scared and because I didn't want people to stare at us in public and think that something was wrong with my daughter. But when we got her casted and the time came close for the fitting I was actually excited to get the process started and this group made a big difference. Just know that you are not alone in this. The time your daughter will spend in a band will actually fly by. My daughter will be a STARband graduate next Tuesday and I don't know where the time went. We are all here to help you and answer any questions that you may have as best as we can. Please keep us updated on how things are going. Guest guest Posted June 4, 2006 Report Share Posted June 4, 2006 I know it can be scary. Our situation was the same as yours (time wise) and we decided to band at 5.5 months. He actually started wearing it 2 days before he was 6 months. Cranial Tech will not make the decision for you but they should have told you if she was a candidate...ie: if she would benefit from a band. Did they take measurements using a caliper? If so, they can give you those numbers so you can know what end of moderate she is and if she has any brachy. My son was 4.5 standard deviations from normal with his brachy. Basically that means his face was very wide for the length (forehead to back of head) of his head. The cranial bones get flattened and make them have wider than normal faces. Good job switching the changing table and crib. My son starting rolling over to sleep on his tummy at 5 months. Once they do this on their own you don't have to roll them back over. But it was too late, his head was already flat. By getting off his back it would not get worse but not get better. We decided to band him. It was a difficult decision especially for my husband. I will tell you, it was the BEST decision ever. It is such a non-event. Our son is not bothered by it at all. It has been four weeks and he adjusted great. His head started rounding out the first week. People in public are just as sweet as ever to him. I know exactly what you are going through. Keep educating yourself and coming here with your questions. You have started at a good age. Know that if you have to band it isn't bad at all and it will be a blip in time compared to a possible flat head forever. I don't want to sound harsh here, but it's not always within your power or control. Medical intervention is often necessary. If you have seen little to no improvement to your daughter's head after trying repo for 2-3 months, then you should consider helmet therapy. I know it's not an easy thing to face, but a Doc band is not invasive. It works similar to dental braces or braces to treat club feet.
