You mentioned you were on lantus and I have been meaning to share this with you for a while but keep forgetting to do so. Dr. Bernstein explains that he has his patients on insulin shots inject no more than 7 units per shot. He prefers the maximum dose to be 4 units per injection. I'll explain why in a minute. So, if you take 9 units of lantus prior to bed, you would take 4 units of lantus in your leg, for example, and the remaining 5, using the same insulin syringe, in your belly, a different body location. He explains that insulin doses larger than 7, again him preferring 4 to be the max but will let people do 7, causes the body to see it as a foreign substance put under the skin and your bodies immune system destroys some of the insulin. When you only inject 4 units at a time, your body doesn't pay attention to it and doesn't destroy it. So, when you take more than 4 to 7 units per shot, you lesson the effect of the insulin since your body destroys some of it and there is no way to keep consistent bgs results with bgs readings. So, by breaking large doses, such as 9 units, into 4 unit small doses using the same syringe but given in different body locations, you fool the body and none of the insulin is destroyed. The benefit to this is you can better control your bgs readings since now you are 100% maximizing the insulin and that gives more stable bgs readings. BTW 9 units to some is a very small dose since some diabetics take upwards of 30 to 50 units per shot. Those large insulin requirements typically are from insulin resistant diabetics, or often times, just to cover quick acting carbs since the diabetic will often times be eating the ADA carb rich diet that is low fat/low meat and high quick acting carbs. * begin rant * As an aside, I can recall staying in the hospital through my childhood and teen years on an ADA (american diabetes association) diet. You want to guess what they served me saying it was perfectly fine and a good diabetic diet? Breakfast typically had oranje juice, 15 grams of carbs, 2 pieces of *raisin bread*, 35 grams of carbs, 1 bowl of cereal, 40 grams of carbs, 1 cup of milk, 11 grams of carbs, and 1 egg. That totals 106 grams of quick acting carbs and they said this was healthy? What about the typical high sugar diabetics run in the morning? And I wondered all the time why my bgs were in the 200 and 300 range a lot? For lunch, they'd feed me a sandwhich, 2 pieces of bread 30 grams of carbs, 1 fruit, 15 grams of carbs, 1 bag of chips, 25 grams of carbs, and a diet pop. That totals 75 grams of quick acting carbs for lunch? And the ADA says that is healthy? So my bgs was 250 from the 106 quick acting carb grams from breakfast then they feed me 75 more quick acting grams of carbs for lunch? hmmmm. It all makes sense why I was a brittle diabetic for years... the ADA and their suggested high qquick acting carb meals and low fat/low meat meal plans and jamming large amounts of insulin in my body to deal with the spikes in my bgs from the quick acting carbs. For dinner, they'd give me pasta, 1 cup 30 grams, 2 pieces of bread and butter, 30 grams, 1 piece of fruit, 15 grams, and a cup of milk, 11 grams of carbs, and a salad and diet pop. That would total 86 total quick acting grams of carbs. So my total quick acting carbs they gave me in a day was 267? Again, all this the recommendation of the ADA (american diabetes association) who still practises this meal plan and are killing diabetics. Strong opinion, I know, but it is true, they have people eat a carb rich diet with an emphasis on low fat/low meat saying it protects the kidneys and heart when all along, the killer of the heart and kidneys is the high bgs, not the fat and meat. Dr. Bernstein and his years of research and his patients have the best blood work/lipid panel/a1c around and eat mostly meat/fat and slow acting carbs like veggies. The high unmanaged bgs is what kills diabetics, not fat and meat. It's your body, try it out. Follow a low carb diet and eat meat/fat and in 3 months, get a lipid panel done and a1c and see the results. * end rant * With regards to dosing 4 unit increments, pretty cool, eh? It is unknown how much carb is in a piece of that-there may be more than 1 or 2 carbs. Re: advice for Harry Puzzling update: Fasting gbs this morning was 88. I ate two scrambled eggs and two pieces of Canadian bacon with a 5.5 ounce of V8 juice, which contains 8 grams of carbs for a total of between 10 and definitely no more than 12 grams of total carbs consumed for breakfast. Two hours later my gbs was 132, a 44 point rise in gbs. Four hours after breakfast my gbs was 116. So I ate lunch consisting of three hefty pieces of roast beef and a big hunk of pepper cheese and a salad with some tomatoes in it, but mostly lettuce and salad dressing of oil and balsamic vinegar. I will check later to see what rise in gbs I get in two hours. Tomatoes and onions are to be avoided since they contain sugar, or, a certain level of quick acting carbohydrates. After all, tomatoes are classified as a fruit, not a veggie. And they are certainly not a slow acting veggie. Think of them along the lines of a fruit rather than a veggie. BTW YMMV may vary with tomatoes... they increase my sugar while others don't have a problem with it. Again, between the two pieces of canadian bacon, containing sugar, and the tomato juice, yes, it is quite possible to jump the sugar 40 points. Looking at this again, you could have had left off the 2 pieces of bacon, contains some sugar, and try the v8 Juice with the eggs tomorrow for breakfast. If your 2 hour post test is more than 120, then you know to not eat the v8 juice and to not eat tomatoes. Bernstein lists tomatoes and tomato juice in the no no list and they are to be foods to avoid. However, like I say, YMMV. Guest guest Posted August 31, 2003 Report Share Posted August 31, 2003 I did not test my gbs two hours after lunch, but instead went on a shopping spree and five hours later I tested my gbs level was 107, so it took five hours for my gbs to fall from 142 to 107, a 35 point drop. For dinner I just ate two hefty pieces of roast beef with mustard and a big hunk of cheese. Two hours later my gbs was 90. After this I took 7 units of insulin. Eat you 1 piece of meat, like a hunk of cheese, and 3 saltine crackers which is a total of 7.6 carbs including the cheese. Each gram of carb will raise your bgs 5 points, in theory for a 150 pound non-obese human, so if you ate 7.5 grams of carbs, your bgs in the morning should be around 127 but will probably be lower than that. I'd hate to see you get low during the night, it wouldn't even be worth it. Be careful. Guest guest Posted August 31, 2003 Report Share Posted August 31, 2003 Three and a half hours after that low 5 carb breakfast, which rased my gbs to 98 after two hours another hour and a half or a total of three and a half hours after breakfast my gbs was once again 86. Then I ate lunch, which was comprised of two hefty pieces of roast beef with mustard, a big hunk of cheese, a boiled egg and a half of another one, and a diet Coke. Oh yeah, I also ate a bite or a spoonful of tuna salad. I estimate that I ate approximately no more than 6 carbs, and definitely no more than 7 carbs. Two hours after lunch my gbs was 96. Three hours after lunch my gbs was once again back to 86. Now I need to get back to walking, since I need the work out. Any ideas on diet, and carb count and exercise? Is the main point here to keep my glucose levels in the normal range 80-120, while avoiding high spikes in the glucose level? A couple of my fingers are getting sore from all those sticks with the needle. I hope I figure out in the near future how to keep in the normal glucose range without spiking high and decrease the number of needle sticks! Guest guest Posted September 1, 2003 Report Share Posted September 1, 2003 is correct on that Harry, check with your doc if it is ok to eat cheese etc. If he states we can try it for a while, then do so and get lab work done in 2 months to see if it is effecting LDL/HDL/tris heart etc. However, I think you will find that by having tight bgs control, it will greatly lower all poor blood work results across the board. How do I know this? Have 1 or 2 cups of slow acting carb veggies per meal, that is about 6 grams of carbs per cup, and those won't effect your bgs to badly since they are slow acting carbs. The reason you need to eat slow acting carb veggies like lettuce, broccoli, cauliflower, spinach, green beans, etc. is those veggies will give you the nutrients and minerals you are not getting from fruits. Again, the veggies I listed a few posts back, have you a helping of them each meal and eat a variety of them. I keep frozen veggies, frozen are the best you can eat since all nutrients are in them, in the freezer and pop open the bag and measure me out 1 to 2 cups and put it in the microwave for about 3 mins and pull out and ready to eat. I toss some cheese on them and down they go smiles. My fingers are not sore at all, are you alternating like I explained a while back? Recall you use your fingers like this: left hand monday pointer tuesday middle wednesday ring thursday pinky now you switch to right hand Friday middle finger saturday ring finger Sunday pinky And the pattern is like this on each finger... 3 pricks down the left side of the finger spread out, 3 pricks down the right side of the finger spread out, and 2 across the center but not on the tip a little below the tip. If you follow this pricking day/pattern, your fingers will not be sore at all. If they are, then you might be pricking your fingers to hard. They should not bruise etc. and if they are, you are pricking to hard. Guest guest Posted September 1, 2003 Report Share Posted September 1, 2003 yes, most diabetics don't eat 30 carbs a day so you have to find what you are comfortable with, or I should say, the level of carb you can eat and maintain bgs of 80 to 120 prior to meal and 2 hours later 120 or less etc. Read my Atkins/Bernstein post so you can see how to use the Atkins theory to determine max carbs per meal. You may have to start using humalog, a quick acting insulin, to cover your carbs. That is, if you want to eat more carbs, with humalog, you can eat quick acting carbs since it will cover them but you run the risk of feeling bad from to many carbs and also the risk of eating like a crazy fool LOL. BTW 1 unit of humalog per 15 grams of carbs, quick acting carbs that is. Humalog is quite powerful and should be used with quick acting carbs, that is why it was created smiles. Talk that over with your doc. Show him your results and explain you'd like to increase your carbs and discuss with him the possibility of using humalog to cover the carbs. Again, 1 unit per 15 grams of quick acting carbs. Explain to him this so he understands that you know how to carb count with insulin. I'd suggest no more than 90 grams of carbs a day, 30 per meal, and if that proves to be unmanageable, then do 25 per meal and so forth. Re: advice for Harry is correct on that Harry, check with your doc if it is ok to eat cheese etc. If he states we can try it for a while, then do so and get lab work done in 2 months to see if it is effecting LDL/HDL/tris heart etc. However, I think you will find that by having tight bgs control, it will greatly lower all poor blood work results across the board. How do I know this? For instance, yesterday, I ate a Bocca sausage with a hot dog bun for lunch. As I knew we were going on a long hot walk, I took no extra insulin (I am on a ppump). By the time I got to the grocery store, I was going into reaction and had to drink a can of juice (6 oz.). When I returned home, my BS was 106. Obviously, when I go on a long walk in the heat of the day, I need more carbs! As this is not a daily occurance, I do not want to decrease the amount of hourly insulin I am getting with the pump. Re: advice for Harry yes, most diabetics don't eat 30 carbs a day so you have to find what you are comfortable with, or I should say, the level of carb you can eat and maintain bgs of 80 to 120 prior to meal and 2 hours later 120 or less etc. Read my Atkins/Bernstein post so you can see how to use the Atkins theory to determine max carbs per meal. You may have to start using humalog, a quick acting insulin, to cover your carbs. That is, if you want to eat more carbs, with humalog, you can eat quick acting carbs since it will cover them but you run the risk of feeling bad from to many carbs and also the risk of eating like a crazy fool LOL. BTW 1 unit of humalog per 15 grams of carbs, quick acting carbs that is. Humalog is quite powerful and should be used with quick acting carbs, that is why it was created smiles. Talk that over with your doc. Show him your results and explain you'd like to increase your carbs and discuss with him the possibility of using humalog to cover the carbs. Again, 1 unit per 15 grams of quick acting carbs. Explain to him this so he understands that you know how to carb count with insulin. I'd suggest no more than 90 grams of carbs a day, 30 per meal, and if that proves to be unmanageable, then do 25 per meal and so forth. Your basal drip is also geared towards your total carb intake so if you are going to adjust carbs down, you may have to adjust your basal/bolus amounts which you know. I adjusted it today,k so will see what happens tomorrow morning. I see my doc next week, so will talk with him about decreaseing the afternoon basal rates so I can eat less carbs. What does it look like? Maybe it has another name in America? Have someone Look on the box, if possible, and see how many carbohydrates are in a serving size of three of those cookies. You mentioned exchange and that flags me that you are on an exchange diet, typical of the ADA, but if that is working for you then that is good. Many find the high carb diet of the ADA, or an exchange diet, makes their bgs readings high and hard to control. I gave up on the doctor reducing my dosage, so I did it myself. Reducing my Lantus from 7 units per night to 5 units per night. This worked out well for a couple of days, so I decided to reduce my oral blyburide dose from 20 mg per day to 10 mg per day, and all I could get was high readings practically all the time. What I mean by high gbs are readings between 150 and 200. So for the past 4 days I have gone back to the original prescription of 7 units of Lantus at night and the glyburide 10mg twice per day. I am back at fasting gbs's of 90 to 100 in the mornings. Also another phenomena I noticed was where my glucose readings seemed to stand still for a while. For instance I would have a gbs of 173 and two hours later without any carb consumption I would have a gbs reading of 172, which meant that something was not working. It seems to be a little more complicated being a type 2 diabetic and regulating the amount of insulin, namely Lantus and oral medications glyburide, which stimulates insulin production by the pancreas. I remain on approximately 45 grams to 60 grams of carbs per day. Trying to maintain a narrow range between 85 and 120 is tricky business. I feel good presently. I just like to avoid those low gbs readings, because in my case when I have a gbs of 75, I feel fine, but a gbs reading of 68 and below, I feel weak and shaky, so I like to avoid those lows. Also I have noticed that I was awakened last night with a racing heart with a pulse rate around 120 for no apparent reason, unless it relates to possible congestive heart failure. The edema in my legs has considerably declined and I would judge that the edema is no more than a 1 plus, rather than the 4 plus which I had recently. So I am still experimenting with myself, and hope to get better control in the near future. Any suggestions are appreciated. First, I can explain the readings of 175 to 172 standing still and not going down with no carbs being eaten. Your oral meds help take care of the carbs, not the lantus per say, and those oral meds help even with small amounts of non quick acting carbs. I think what you were experiencing was the ongoing high sugar that you probably had going on for the year or more you didn't test. That is a scary thought my friend. You see, once your high, it can be hard to break the high sugar since each meal can keep feeding the high sugar if you don't have enough medication to keep you in your target range of 90. To prevent the feeding of a high sugar, meals should be spaced out 4 to 5 hours and drink lots of water when you are high sugared. Water helps thin the blood and makes your cells more responsive to insulin or oral medications. Lantus is a basal insulin, which means, it mimics the pancreas keeping a constant flow of insulin in your blood stream mimicking the constant periodic drips of the pancreas. It is designed to help keep a straight line effect of insulin in the blood. It is not designed to deal with carbs. I think your dose of 9 lantus is fine. Remember, let your meter tell you what the best blend is for your body. Your meter is your best friend. However, like you suggested, your oral meds were designed to help your pancreas create more insulin, or encourage it to, to handle the carbs. I strongly suggest to get on humalog insulin, a quick acting insulin, to deal with carbs if you want to ease up the work load on your pancreas. If you don't want to take a shot prior to each meal, I understand, but it really isn't that bad and can help you lesson the work load on your pancreas and possibly help preserve the remaining beta cells you have working so if they do finalize a cure for type 2 diabetics in 5 years or so, you could be eligible for the cure. Also, if your bgs is 68, then eat only 1 glucose tablet since it will bring you back up to 88 your target range YMMV you might need two tablets. In closing, I think you and me have found a system that works for your body... 45 to 60 grams of carbs a day, 9 units of lantus, and your oral meds. If that regimend keeps you 90 prior to a meal and 120 or less 2 hours past a meal, then that is great! The racing heart at night could have something to do with your heart condition, but it could also be a low BS at night. Did you do a blood sugar then? When I wake up like that, I am often perspiring also and it always means low blood sugar. Also, if y ou BS is staying up, it might mean you need some fast acting insulin to bring it down. I wondered about the blood sugar being low at night when I awake with a rapid heart beat, but I do not wish to awake my wife in the middle of the night to test my gbs. So I will decline to do that unless the problem persist. I may have to resort to buying a glucose monitor that a blind person can use, but they are too expensive. Of course I forgot to take my 10 mg of glyburide along with breakfast, so this may explain the rise of 91 points with only 6 grams of carbs for breakfast. So I took my glyburide, and I will measure the gbs before lunch to see if it falls within the narrow 80-120 range. How would one dose on humalog for 6 carbs at bfreakfast that shoots the gbs up 91 points two hours later? Oh well, I will just keep on sticking my finger 8 and 9 times per day for those gbs readings until I get a grip on these medication dosages. BTW I am on 7 units of Lantus, not 9 units.
