We don't know beyond one small study by Piscitelli which showed a decline in trough indinavir levels in HIV-Negative volunteers taking monotherapy IDV. The authors did not feel that this was a clinically significant finding--i.e., the effect won't impair the ability of IDV to work (esp. in the context of a cocktail). But we don't know. Currently, a study of MT in HIV-HCV coinfected people is being proposed--maybe we'll get some answers. In the meantime, MT may help if your liver enzymes or histology are out of whack (or if you're glutathione is low but it is hard to know if that is the case). I think the modest risk of an effect on PI/ARV level is outweighed by the potential benefit for improving liver function. I think if I had to take ARV and with my HCV, I'd take milk thistle. M. I'd recommend you start milk thistle on the same day you get bloodwork. Another study in HIV-negative volunteers found no serious effect of milk thistle on indinavir levels. This suggests it should not mess with your antivirals but we don't know. Happily, it looks like FIAR will have its first clinical study starting soon, looking at milk thistle in people with HIV and hep C. The study will be taking place at Mt. Sinai in New York. As to side effects, they are pretty rare. Gastrointestinal effects may occur; they're usually pretty mild. Rash can occur in some people. Nausea and vomiting have been reported--but these are all very rare occurrences. Still, forewarned is forearmed. It has been tested in many clinical settings and found to be extremely well tolerated. For brands, a good question. I do not know about the Solgar brand for sure. Thistlyn has been used in a variety of tests and may be one of the best. Made by Nature's Way. Other good companies that I think are reputable include Thorne Research, Phytopharmica, Pacific Biologics, Herb Pharm--but I'm not sure if they make milk thistle that's easily obtainable. I have used the Solgar brand and it seems OK. Consumerlabs has not yet done a test of milk thistle so far. Once the study gets its FDA IND cleared, we'll be underway and a more formal announcement will go out. When we do, we'll be testing the batch and lot independently to assure identity, potency and purity. Meantime, we have to wait for the GMP rulemaking to become effective, however, I think a lot of the companies are trying to do a decent job. I am not familiar with the individual herbs but know that it is said that there is a "cure" for everything in the rainforest of the amazon! It featured alternative modes of healing from all over the world, including Hawaii. Some of what we saw was about the curanderas from Mexico and South America. (I just realized that Milagro de la Selva means Miracle from the Jungle.) One movie showed about the indigenous community in the jungles of Peru and some of their incredible herbs. I loved how the researcher traveling and studying the healing herbs and the culture of the medicine men only created documentation for them to keep in their own language, so that it would not be exploited by wealthier nations. He took a lot of flack about not publishing the results in peer reviewed journals, but he didn't care! There is a company called Amazon Rain Forest that sells things like vitamins, supplements, chocolate, and tea using these herbs. Their skin care line seems really pure and nice, but it's a network marketing system which I'm not quite so fond of. I have been to the Ecuadorian Amazon rainforest.
