His response was to stop all meds and retest in 2 weeks...

rheumatic liver enzymes

Hi Mike,

What was your doctor's respnse to these levels? Did he recommend anything

to bring them down?

If they're still in the normal range, there's

nothing to worry about right now. However, they will

want to keep a close eye on them because one

increase can become a trend and they may keep

getting higher.

My liver enzymes took a pretty big jump very

suddenly, from low-normal to a fair bit above

normal, and so they stopped my methotrexate

immediately. But my doctor and my rheumy have both

said that there is no immediate concern about liver

damage. If they stay high there will be concern, and

if they were extremely high then they would be doing

a biopsy or something; but as it is, they assure me

it's okay. (Right now they've dropped a bit but are

still above normal and we're waiting to see what

happens.) So I think you have nothing too much to

worry about -- just as long as they keep an eye on it.

My doctor told me he would really become concerned about the liver

enzymes when they raise to about 300 and stay there. It damages the

liver if the enzymes are too high for a long period of time.

This is what my last blood results were:

ALP: Mine tested at 129 Normal Ratio: 50 - 136

LDH: Mine tested at 155 Normal Ration: 100 - 190

(There are a lot of other test done on my liver, but none showed a

high number.

ALP is Alkaline Phosphatase. It is produced in the liver, bones, and

some in the intestines and kidneys. If ALP is raised it is usually

from various reasons, such as bone disease, cancer, jaundice, etc.,

or even medicines we take such as our PA meds.

WebMD says the normal range is 38 to 126 U/L while my blood tests

shows the normal range to be 50-136. Hmmm...

LDH is Lactic Dehydrogenase. This enzyme produced in the liver helps

produce energy by converting LACTID ACID to pyruvic acid during the

metabolic process. It is present in almost all of the tissues of the

body and becomes elevated in response to cell damage. Many diseases

cause LDH to raise including heart disease.

According to WebMD the normal ratio is 45 U?L to 90 U/L. Now on my

copy of my blood work the normal range was 100 to 190, so this is

something I guess I should ask the Doc about.

I just went to my PCP last Tuesday because of my concern with the

Placquenil. He put me back on Sulfasalazine 500 mg. twice a day, and

raised my Bextra to 20 mg twice a day. They did a liver enzyme test

that day, and I go back in 6 weeks to see what the damages are.

Hopefully, the test will show a low increase in liver enzymes, and

I'm praying there will be no increase at all. I can hope, anyway.

It has been raining all weekend and today, Monday, and I'm hurting,

but last week was the first week in a long time I could move without

wanting to cry. Maybe it's all in my head, the relief. But if it

is, I'll take it!

Good luck! Maybe someone with medical knowledge can explain it

better!

Dear Debra,

There are more than one way to do tests and each lab establishes its

normals. You have to go by the normals as stated by the lab the tests were

done at. The enzymes themselves do not damage the liver, they are produced

by the damage. Since your levels were in the normal range for the lab where

they were done it is probably best that you don't worry so much, just wait

to see what the repeats are. Anne

I am in the same boat as you.

My doc called last week and told me to STOP all abx because my liver enzymes

were elevated to 90.

In the mean time to aid with detox he was very adamant that I go on Nature's

Sunshine 90 day health challenge, supplement with Milk Thistle, juice with

apples, carrots, lemon and beets, epsom salt baths, and drink lots of water!

I will have new blood levels drawn on my next visit and see if I will start

abx again...I am really gun shy now!!

I think once your liver enxymes come down you should probably STAY on this

type of liver cleansing the whole time you are on abx...it can't hurt and

can only HELP!

Hi Dr. Chiu,

Would you then substitute a different antibiotic - like clarithromycin

- or give the patient a complete break from all antibiotics for a

period of time?

As you know, I have liver problems myself, but they are AI not from the

mtx which I can't take.

The only person I know who took Lunesta had to quit because of short

term memory problems it caused. I hope you don't experience that side

effect, but without good sleep, our memory isn't that good anyway, is it?

The nurse called today to let me know my enzymes are high again. So,

I have to lower the mtx from 0.6 to 0.4. I could tell today with the

mtx and steroid shot, the joints were feeling better. I am always

afraid to say, " I'm feeling better " I jinx it. I hope the mtx will

still work with the lower dose, I'm praying I will be approved for the

Enbrel soon. They called 2 prescriptions in for me. The sleep med

didn't work they gave me last week, so I will be trying Lunesta,(not

sure how you spell it). Also,the one that takes the place of Folic, I

don't know the name of it. I hope everyone has a peaceful night.

I am sorry to hear that. So they are starting you on Leukovorin. I tried

it with folic acid for the MTX side effects. It did not work for me. I hope

you get some relief from side effects.

Nina,......The enzymes kind of worry me. Last year, my other rheumy

sent me to a liver spec, everything seemed normal. I have a weird

pain all the time in my liver area. I can't do much of anything, the

pain will start in that area. Thanks so much for the support, T

Upper right quadrant pain, right? That's the liver. The liver actually

has no pain receptors, but the lining surrounding it does. I have the

same pain. It used to be really bad, but since my alkaline phosphotase

went from 269 (should be less than 20) down to 189 and now is at 145,

the pain is not as bad.

Have them keep checking your liver. Have they checked you for Primary

Biliary Cirrhosis? It's an autoimmune disease although when people hear

" cirrhosis " they think alcohol. It's frustrating when they do that. I

was diagnosed with Fibro, Sjogren's Syndrome and PBC all at the same

time. You can't live without your liver and transplants are hard to get

so please do keep an eye on your liver. You might even try another

liver specialist. Someone who does liver transplants or is in a

practice with someone who does. The last gastro doc I went to kept

asking me, are you sure it's PBC and not PSC? It's so rare, he had a

hard time believing I had it even though bloodwork and a biopsy has

proven I do. Everyone who has PBC has Sjogren's Syndrome, though not

everyone with Sjogren's has PBC. And it tends to be inherited from your

mother. They're actually doing a study on it with mothers and daughters

because so many daughters and mothers have been diagnosed.

Make sure your doc checks his prescription book when he prescribes any

meds for you to see if they are processed through your kidneys or

liver. When there's a choice, my doc always prescribes what is

processed through my kidneys. I can't even take Tylenol.

Hope you find a doc that can really help you.

Yes, the pain is in the right quadrant. This has been going on for

awhile. I've had pain in my lungs too, like burning. The GP told me

I had bronchitis, and then when I went back, it was whooping cough.

When I mention the pain in the area of my liver, she tells me it's

probably because of my coughing. I need to do something about it, I

am very worried. I am just fatigued all the time, I can't get my

butt out of the house. I'm so sick of doctors. I have several

problems that could be serious, but I just put them on the back

burner. When I went to the liver doctor, they just ran a lot of

blood work, and I had a ultrasound of my liver, which was OK. Thanks

I know the feeling. I'm sick of doctors as well.

Fatigue is a symptom of PBC as it is with many AI diseases. There are

specific blood tests for them to do when trying to diagnose PBC and you

have to have a liver biopsy. An ultrasound is not going to show

anything they need to diagnose.

crossed. I hope they find something simple and minor that is causing

your problems.

Don't do too much. You will just pay for it tomorrow, if you're like

me. Just do a little here and a little there. Be kind to yourself!

Jolene, when I started Arava, my liver enzymes went up. My rheumy said

that they don't worry until they're 2-3 times the normal level, I

believe it was. So I don't think you need to worry. Wait and see what

your rheumy says.

My Rheumy doesn't do anything about meds until blood levels are 3 times as

much for 3 months straight. I'd say if your Rheumy does anything about your

MTX, I'd question him/her to find out what and why. You can be your own

advocate when dealing with things of this type if you do some homework. Good

When my liver enzymes were twice normal for two months I had to stop

my NSAID and my Zocor for over a month. Being off my NSAID really

made my joints more painful.
