I now take Isentress (raltegravir) and Truvada (tenofovir/FTC). At first I saw a radical improvement, with my CD4 count going up to 94 and my viral load dropping to around 1,000. After four months my viral load is undetectable but my CD4 count is rising very slowly. I eat right, exercise, sleep enough, work hard, enjoy life, and take several vitamin and mineral supplements. How can I expect to fare relative to someone who started treatment earlier in their HIV infection? Can I still plan for retirement? Dealing With Post-Diagnosis Stress: Can I Smoke Occasionally if It Helps?

After being diagnosed with HIV in the fall, I quit smoking by using a nicotine patch. I was doing well until a few weeks ago when the cravings began to reemerge. My infectious disease doctor is great and my HIV med regimen is working well, but I'm still adjusting emotionally to being an HIV-positive person. The thought of never smoking another cigarette is depressing me. Considering the amount of stress I'm dealing with, would light smoking be OK if it allows me to keep my sanity?

How Can Sex Be Even Safer in a Mixed-Status Partnership?

I'm in a very committed relationship with a wonderful guy who happens to be living with HIV now. His viral load is still low and his CD4 count is close to 500, so his doctor hasn't advised him to start treatment yet. We have as safe sex as we can -- never penetrating without a condom and usually just jerking off or coming on unbroken skin. We also either use a condom during oral sex or are careful there's no pre-cum present. I've consistently tested HIV negative. What else can we do to protect one other's health?

What Kind of Support Group Is Right for a Senior Living With HIV?

I'm 68 years old, I've been HIV positive since the mid-'80s and have done relatively well living with HIV. I have avascular necrosis, which has pretty much destroyed both my hips, as well as stage IV lung cancer. I was referred to a mental health provider who handles people living with HIV/AIDS, but they don't work with seniors. It seems that once you pass 65, you are supposed to be dead, healed or not in need of mental health services. Someone suggested a geriatric group in a distant part of my city. Do I just forget about the AIDS part of my situation?

Diagnosed With HPV: Do I Need to Stop Having Anal Sex?

I'm a 44-year-old gay man who's been taking Atripla (efavirenz/tenofovir/FTC) for the past three years, with an undetectable viral load and no side effects. I eat very well, exercise regularly, don't drink and have never smoked. I'm also in a "magnetic" monogamous long-term relationship and always practice safer sex. However, I recently learned that I have several strains of human papillomavirus (HPV). I haven't developed anal cancer, but after all my hard work at taking control of my health this latest news has stressed and slightly depressed me. Is there anything else I can do to prevent anal cancer? Should I avoid anal intercourse altogether?

I've been taking Truvada (tenofovir/FTC) and Viramune (nevirapine) for 10 months. I'm going to the Dominican Republic later this month, and I'm supposed to take anti-malaria medications for the trip. My Nephew May Be Dying: Can I Help Him Help Himself?

My nephew is 24 years old and has been HIV positive since he was 9 years old. He's been staying at an HIV/AIDS house since June 1 because he needed care I couldn't give him. He hasn't taken his meds for 1 1/2 years because he says it messes with his head, but he has lost so much weight and he looks bad ... I was told if he doesn't take his meds he could die. If he starts taking his meds again, would it be possible for him to live longer? How can I help get him to take them? -- rlsunshine

What's the Risk of Long-Term Prednisone Use?

I have an undetectable viral load and 350 CD4 cells, and I take Intelence (etravirine), Isentress (raltegravir), Norvir (ritonavir), Prezista (darunavir), Ziagen (abacavir) and blood pressure meds. For the past year I've had a persistent, itchy rash. My doctor thinks it's a side effect of one of my meds but isn't sure which one. Small doses of prednisone take the rash away, but when I stop the prednisone the rash always comes back. For how long can I take the prednisone? Will it mess with my immune system?

Are High Viral Loads Common in Recently Diagnosed Individuals?

I was diagnosed with HIV six months ago. My initial CD4 count was 256 and my viral load was nearly 2 million! I've never heard of anyone with a viral load that high. My doctor seemed to think it could be the spike that's often seen with a recent infection, but he's never seen a viral load that high either. It concerns me that I may have an aggressive strain of HIV. Are these numbers common with a recent infection or could they indicate something more serious?

What's the Link Between Joint Pain and Selzentry?

I was experiencing extreme pain in my finger joints, elbows, shoulders, back and knees, to the point where I could barely function. I was taking Selzentry (maraviroc, Celsentri); since I stopped taking it about three weeks ago, the pain has pretty much subsided. Naturally this leads me to believe Selzentry was the culprit for my pain. What do you think? Is joint pain a common side effect of this drug?

Is It Really Better to Start HIV Meds ASAP?

I'm a 36-year-old guy, newly HIV positive and really struggling to understand the subject of when to start HIV treatment. You've mentioned on this forum in the past that you believe evidence will emerge that starting HIV meds earlier is better for preserving the immune system. I've also read that being on meds for too long isn't good either and that it's better to just start taking them at the recommended time. How do I figure out what the best thing is for me to do?

Tips for Managing Anxiety and Depression?

I was given an AIDS diagnosis in 2008, at age 32. I was very ill and in the hospital for a week. Now my CD4 count is around 750 and my viral load is 12,000. I've had mild depression for years but since my diagnosis it's gotten worse. At first I had bad panic attacks. I started an antidepressant and a beta-blocker, and they helped a lot. I have seen a therapist and was diagnosed with post-traumatic stress disorder (PTSD). My biggest problem now is anxiety when traveling. I have this irrational fear that something bad will happen to me far from home, and then I panic. Is there anything I can do when I'm on my own (not in a therapy session) to help manage episodes like this?

Is My Low-Testosterone Treatment OK for People Living With HIV?

I'm HIV positive, my CD4 count is 450, my viral load is below 20 copies and I take Epzicom (abacavir/3TC, Kivexa) and Isentress (raltegravir). I was recently diagnosed with secondary hypogonadism (low levels of certain sex hormones). My endocrinologist prescribed Femara (letrozole) to increase my testosterone level. How Can I Quit Smoking Cigarettes?

Since the 4th of July, I have been trying to get myself independent of smoking cigarettes. Although I have yet to kick the habit completely, I have been able to cut down. I used to smoke two packs a day, but now I'm down to half a pack a day. That's a big drop for me. You see, I've been smoking since they were only 48 cents a pack. I've had some issues putting them down before, tried pills, patches and other things that didn't work. It's kind of funny how we know we're doing all we can to make ourselves healthy, yet some of us continue to hinder our health with these things. Does anyone have any quitting advice? -- alive2

Viral Load Low but Detectable: What Should I Be Keeping an Eye On?

I was diagnosed with HIV in March 2009. My viral load is 892 and my CD4 count is 952. My viral load tends to hover in the 900-to-1,000 range, and my CD4 count tends to stay in the 700-to-800 range. Is this average progression? Are my numbers likely to remain stable for a while longer? What factors might upset this trend? Should I think about starting HIV meds soon?

STRANGE BUT TRUE

Risk of Sticking Random Objects Where the Sun Doesn't Shine?

I was alone and being stupid, and I put an old toothbrush and a sun umbrella tube in my anus. I did it a few times, and bled once or twice. I only washed these objects with water before I stuck them up there. Has anyone ever become HIV positive in this way? I'm killing myself inside with this question!

I knew I was poz on some level, but put off getting tested because of ignorance on my part. In October I got shingles, April oral thrush. It was actually the thrush that made me get tested (poz, of course). ... I was diagnosed May 2 and got my first labs May 31, only to find out my CD4 count was only 12. Been taking Atripla for 25 days now with a hospital stay for CMV virus. .... What if the meds don't work? ... I don't wanna be that sick again. I tested HIV positive through a clinic and found an infectious disease doctor who put me on Atripla (efavirenz/tenofovir/FTC). I'm a gay 16-year-old. My boyfriend recently found out he may be HIV positive. The doctor told him his viral load is very low and he needs to retest in six months to see if he really is positive. We had unprotected sex before his results; I topped him but did not ejaculate inside of him. I've tested negative so far. I'm very depressed; I was being young and stupid. I know there's a drug interaction between Kaletra (lopinavir/ritonavir) and the erectile-dysfunction drug Cialis (tadalafil), for which I have a prescription for 20 mg. Any idea how much I should start with when using Cialis? Should I split the pill in half or just take the full dose?

What Do Studies Say About Taking Epzicom and Reyataz Without Norvir?

How acceptable and prevalent is the combination of Epzicom (abacavir/3TC, Kivexa) and Reyataz (atazanavir) without Norvir (ritonavir)? I'm currently taking all three. My virus is undetectable and has been for several years. My doctor is quite conservative and not very willing to change regimens. The side effects and expense of Norvir are my concerns. HIV Treatment Newbie: How Do I Take My Meds Correctly?

I've been on Atripla (efavirenz/tenofovir/FTC) for about two weeks with minimal side effects, but now I'm worried about making sure I take it correctly. How important is it to take at the same time every day? How much leeway do I have? Can I not have food two hours before and one hour after just because of side effects or because of the effectiveness of the medication? Can I take my meds after or while drinking alcohol? What if I miss a dose?

I was diagnosed HIV positive in May. From what me and my doctor can conclude, I became infected in February. ... The last three months have really been a struggle. I started taking an antidepressant, and I have taken Ambien to sleep every night for the last three months but I want to stop. I feel like my life is out of my control and that drives me crazy since I am a control freak. I also have some paranoia when talking with others ... I'm always really worried that others are judging me because I'm HIV positive.

Needle Poke at Work: What's My Next Step?

I work at a state prison and caught two inmates on a cellblock tattooing each other with a homemade machine. I took it from them and about 20 seconds later I accidently poked myself with the needle at the end of the tattoo "gun." I can't remember if it penetrated my skin or not. It hurt a little bit but did not bleed or leave any mark. There's no way my work can tell me the HIV or hepatitis status of the two inmates. Is this a transmission risk or am I just being paranoid? What, if anything, should I do next?

Gay Guy Gone Straight! Did I Put Myself at Risk?

I am 30, gay, and have always played safe in the past. I hurt my back and my doctor sent me to a physical therapist. I arrive at the place, and a nice girl takes me to the room where I get a very therapeutic lower back massage. But then she starts to play with my penis! At first I was hesitant but figured "why not, it feels good, the hell with it." So basically, I strayed from gay and got a hand job at a massage parlor, with oil. I couldn't see if she had any cuts or lesions on her hands. Am I at risk for HIV from this? How about other sexually transmitted diseases?

I'm a gay 16-year-old. My boyfriend recently found out he may be HIV positive. The doctor told him his viral load is very low and he needs to retest in six months to see if he really is positive. We had unprotected sex before his results; I topped him but did not ejaculate inside of him. I've tested negative so far. I'm very depressed; I was being young and stupid. My parents would kill me if I became HIV positive. I could be thrown out of the house, because my father's a pastor. I hate myself for being gay, and nobody understands but my boyfriend. I have been HIV positive since 1985. I take protease inhibitors and have advanced osteoporosis. My doctor gave me testosterone and there is a little improvement. Before my diagnosis, I was a very spiritual person. Not religious, but spiritual. I found out my status on June 21st which apparently is St. Lazarus's feast day, who is often petitioned for the poor and sick (especially those with HIV/AIDS). And before finding out my own status, I had been praying to this saint for an HIV cure. I don't know if that's supposed to mean anything or if I'm just grasping at straws. Maybe I'm trying too hard to look for signs of God, when it could be just a coincidence. Anyway, I've learned that most HIVers are atheists. Are Loose Stools and Yellow Semen Side Effects of my Medications?

For the past two months, I've been taking Reyataz (atazanavir), Norvir (ritonavir), Epivir (lamivudine, 3TC) and Retrovir (zidovudine, AZT). My CD4 count has been slowly improving. However, for the past three months now, I've frequently been having loose stools and have noticed that my semen is yellowish. Is this normal?

Before my diagnosis, I was a very spiritual person. Not religious, but spiritual. I found out my status on June 21st which apparently is St. Lazarus's feast day, who is often petitioned for the poor and sick (especially those with HIV/AIDS). And before finding out my own status, I had been praying to this saint for an HIV cure. I don't know if that's supposed to mean anything or if I'm just grasping at straws. Maybe I'm trying too hard to look for signs of God, when it could be just a coincidence. Anyway, I've learned that most HIVers are atheists. I was wondering if anyone here had some kind of religious or spiritual beliefs that were affected by HIV. -- PageofCups

A "Normal Life" on HIV Treatment: Just a Fairy Tale?

It seems from my last lab test results that I'll have to start HIV treatment soon. I've done my research and frankly, I have to wonder whether it's worth it. Of course, without meds I may be more likely to become very ill and even die. But I found no pill that didn't have some potential life-threatening side effects; and the ones that aren't life threatening still seem to spell a life of misery: vomiting, diarrhea, psychiatric symptoms ... I don't think I'm the kind of tough guy who could endure all that. Do the stories of people living "normal" lives on HIV meds have any truth to them at all?

I'm 37 years old. I've been HIV positive for the past five years. My CD4 count as of July was 830 and my viral load is 570. Two weeks ago I was diagnosed with genital herpes. What Can Milk Thistle Do for My Liver?

I've found conflicting information about the herbal supplement milk thistle (Silybum marianum). Some reports claim that it can help the liver, may lower cholesterol, and may reduce insulin resistance. I was diagnosed on August 3, just over four weeks ago ... and I've been keeping it a secret. ... I don't have close friends or a boyfriend. ... I can't tell my family as I hear comments they make about HIV-positive people.

When I find a partner I think that will be the first person I tell, because I wouldn't want to infect someone knowingly. The thought of knowing I will never have sex flesh-to-flesh for the rest of my life kills me. I guess with time I'll accept. Can someone share how they've coped with that? -- shalomt

Viral Load's Gone Down, but CD4 Count Is Lagging: What's Going On?

I was diagnosed with HIV two months ago with a viral load of 21,000 and a CD4 count of 140. I was put on Sustiva (efavirenz, Stocrin) and Truvada (tenofovir/FTC) and after four weeks my CD4 count went up to 190 and my viral load dropped to 97. I had a second blood test after nine weeks on meds and my viral load is undetectable, but my CD4 count dropped to 180 and my CD4 percentage went from 13 from 18. Can you help me understand this?

Do I Really Need Both Truvada and Sustiva as PEP?

A few days ago I had receptive anal intercourse and the condom broke. I went to the ER and the attending physician put me on a PEP (post-exposure prophylaxis) regimen consisting of Sustiva (efavirenz, Stocrin) and Truvada (tenofovir/FTC). Just learned of Dr. Bob's passing and I am devastated. I read his answers and your answers daily for a source of strength of knowing that you were both still here. I do not know how to get that strength now. Dr. Bob wanted to "be here for the cure"; now that whole idea seems like a cosmic joke. Atripla to Isentress and Truvada: What to Try Before Considering a Switch?

My partner has been on Atripla (efavirenz/tenofovir/FTC) awhile and has mostly tolerated it well. However, he wonders if he should switch to Isentress (raltegravir) and Truvada (tenofovir/FTC) due to some trouble falling asleep at night. Should he do something like change his dose times first, or just go ahead with the med switch?

Can I Take My Old Regimen Until I Can Get a Refill of My New Meds?

I'm a 29-year-old man and I've been HIV positive for two years. My viral load is undetectable and my CD4 percentage is less than 18. I was on Atripla (efavirenz/tenofovir/FTC) when I first started HIV meds, but because of side effects I switched to a regimen that includes Epzicom (abacavir/3TC, Kivexa) and Reyataz (atazanavir). It has been fabulous for me -- but today I ran out of meds. My doc is on vacation and I have no way of getting them. Should I take my old Atripla for two days until I can get my regular meds on Monday morning?

Should I Keep Taking Valium for Chronic Anxiety?

I was diagnosed with severe depression and general anxiety disorder three years ago. I just found out I'm HIV positive this past August 8 and know I was infected two years ago, though my virus has not progressed. My doctor told me I need to see a psychiatrist and get my depression and anxiety under control or it could have a negative effect on my HIV health. I've been taking Celexa (citalopram) for depression, as well as Valium (diazepam), for over a year and a half. While Valium is still helping my anxiety, Celexa no longer works for me. Would staying on Valium and switching antidepressants be safe for me?

Where's the Majority of My Virus?

I'm newly diagnosed HIV positive, seven weeks into treatment and with fingers crossed on the road to undetectable status. I understand that only 2 percent of the virus is in the blood. Where is the other 98 percent? Also, if the virus is undetectable in your blood, how is the assumption made that it's not undetectable in these other places?

Could I Have Gonorrhea of the Throat?

I've gotten two blowjobs recently. I know the chance of getting HIV from a blowjob is very small. Several weeks after the last blowjob I got tested for all sexually transmitted diseases (STDs) and HIV. All tests came back negative. But last Friday I got a call from the woman who blew me, saying she has gonorrhea in her throat. I know that she hooked up again with her ex-boyfriend. But what if she already had gonorrhea in her throat when she gave me those blowjobs? Could I have it too, even if I tested negative after my last contact?

I was on Atripla (efavirenz/tenofovir/FTC) when I first started HIV meds, but because of side effects I switched to a regimen that includes Epzicom (abacavir/3TC, Kivexa) and Reyataz (atazanavir). It has been fabulous for me -- but today I ran out of meds. My doc is on vacation and I have no way of getting them. Should I take my old Atripla for two days until I can get my regular meds on Monday morning?

Should I Keep Taking Valium for Chronic Anxiety?

I was diagnosed with severe depression and general anxiety disorder three years ago. I just found out I'm HIV positive this past August 8 and know I was infected two years ago, though my virus has not progressed. My doctor told me I need to see a psychiatrist and get my depression and anxiety under control or it could have a negative effect on my HIV health. I've been taking Celexa (citalopram) for depression, as well as Valium (diazepam), for over a year and a half. While Valium is still helping my anxiety, Celexa no longer works for me. Would staying on Valium and switching antidepressants be safe for me?

Is HIV Multiplying in My Blood?

I was diagnosed with HIV in November 2010. Right before testing positive, I was experiencing night sweats. Now I'm starting to have night sweats again. Does this mean the virus is multiplying in my blood? Has anyone else experienced this symptom? -- Still_here76

Where's the Majority of My Virus?

I'm newly diagnosed HIV positive, seven weeks into treatment and with fingers crossed on the road to undetectable status. I understand that only 2 percent of the virus is in the blood. Where is the other 98 percent? Also, if the virus is undetectable in your blood, how is the assumption made that it's not undetectable in these other places?

Could I Have Gonorrhea of the Throat?

I've gotten two blowjobs recently. I know the chance of getting HIV from a blowjob is very small. Several weeks after the last blowjob I got tested for all sexually transmitted diseases (STDs) and HIV. All tests came back negative. But last Friday I got a call from the woman who blew me, saying she has gonorrhea in her throat. I know that she hooked up again with her ex-boyfriend. But what if she already had gonorrhea in her throat when she gave me those blowjobs? Could I have it too, even if I tested negative after my last contact?
