What Specific Recommendations Can Be Made for the Treatment of

Severe Obesity?

Decisions on what therapy to recommend to patients with clinically

severe obesity should depend on their wishes for outcomes, on the

physician's judgment of the urgency of the need for therapy, and on

the physician's judgment of possible options for therapy and their

probable efficacy.

Patients seeking therapy for the first time should be evaluated by a

knowledgeable physician and provided with sufficient information on

which to make a reasonable choice for therapy. In most cases,

patients should first be considered for treatment in a nonsurgical

program with integrated components of a dietary regimen, appropriate

exercise, and behavioral support and modification. Possible

comorbidities such as hypertension and diabetes should be sought and

treated if not already under treatment. The desired outcomes may

vary among patients and include such indices as a gain in the

quality of life as judged by the patient, reduction of hypertension,

and amelioration of glucose intolerance. A judgment of failed

nonsurgical therapy should be followed by a decision for nonsurgical

therapy in a different kind of program or with a different

therapist, for no further therapy if significant comorbidities do

not exist, or for surgical therapy.

well, I haven't written much lately, but I have found something this summer

that really helped me and Jasmine with our dry scalp. we got a bar of lye

soap at Dollywood and used that to wash our hair. yep, that's right. sounds

gross, but it worked like a charm. and really made our hair nice.

Karac doesn't like to be told "no" either; so if he is wanting another cookie after he has already had two, we say, "How about some carrots? or would you like to go swing?" That almost always works; when he still persist, I say, "Go to your room and get your blanket and watch a movie. Karac has never been a spitter, but he would pull hair or pinch. During those times my therapists would wear long sleeves and a cap. I don't know how one would protect themselves from being spit in the face unless maybe wear a ski mask. smile. LOL, Pat K

My son used to be the same way. He still is at times but not as bad about it. I just had to be persistent and let him know that I am the boss and didn't let him have what he wants all the time. When my son would act like that I would just hold him down in the chair or sit and hold him until he gave up the fight. By doing this, I've let him know that this behavior IS NOT acceptable and I would not let him go until he stops. It usually took 30min-1 hour before he would stop at first, but after I kept doing it every time he misbehaved and got violent like that, it doesn't take as long for him to calm down. I also have a picture schedule for him and his daily routine. I also have pictures that symbolize TIME OUT or No SCREAMING, NO HITTING, NO SPITTING...etc.... This seems to help alot too. ........When he is good I praise him and reward him as often as I can to encourage the good behavior.... Another suggestion, if there is a lot of anger and tension in the home it makes the child's behavior worse. My daughters Dad and I lived together for a while with the the kids and he was an alcoholic. We fought all the time and I was always angry and it seems that since he has left the home, My son doesn't act up as bad...... I also would never give him anything until he asked in a normal or low tone of voice. For instance he'd say "Juice Now!" and scream about it. But I would not give it to him until he'd say "Juice please" and say it in a low tone. It's very hard and time consuming, but it's well worth it in the long run. I hope this helps....... Melinda

A visual reward board for good listening, quiet hands Ect. worked for us. 5 happy faces = a muffin. Works for meltdowns too. Redirecting ( Pat :0) ) is also used with the reward board. For CB we used his Leappad...5 happy Faces= 30 minutes of of book. When he would attempt to pinch but not make or light contact ..grabbed the board and said GOOD CONTROL ! YOU GET A HAPPY FACE ! Works for every situation...going to the store ...take a small note book. CB's shrink gave me this tool.

Hey Tatiane,

My first thought of encouragement is: LOVE YOURSELF!!! I can

understand your frustration, but believe me things only work and get

better if you start off loving yourself and the way you look first.

I dont particularly like my bosy either....but I dont hate it

either. There are worse things....

Whenever you change your eating habits you must be patient.

Dont solely rely on the scale. Take you measurements: bust, thighs,

hips, arms, neck, waist....I bet that youll see some changes. Also,

calculate your body fat% and lean mass. Do a google search and you

will find calculators to help!! Also, DONT weigh yuorself everyday

or every week for that matter. The scale is deceiving. Try

weighing yourself every two weeks at the beginning of the week. And

keep records

This is my first week of BFL, but by far not my first diet. I

have lost and gain wieght. I too wish I could be a size 6 and

confidently wear a bathing suit, but i know that this type of

program takes patience. And what makes it soo good is that its not

a temporary fix, its a lifetime fix!!!!

WE ARE ALL HERE FOR U!!!

Tatiane,

You might try sending in your daily menu and let us take a look at it.

Sometimes that can give you an idea why things are not happening. You might also

try logging your food intake into fitday.com.

But I think your problem is your thoughts, you need to have a positive

attitude, because negative thoughts can do terrible things to your self-image.

In order to end our “war on weight” we need to start measuring our self-worth

beyond the scale. We need to appreciate our physical body by feeding it with

whole, natural foods. We need to reclaim responsibility for our lifestyle

choices and start moving our bodies in the way they were intended. We need to be

inspired. Inspired is to be in spirit, which is the intangible measurement of

any health and fitness accomplishment.

Tatiane, what inspires you to take care of your health? We can't tell you

these things, ultimately you have to decided when you are " sick and tired " of

being disgusted with yourself, and from there you will find the motivation.

I promise you if you follow the BFL lifestyle you will lose weight, and you

can do it!!

Hi Tatiane,

Be patient with yourself. Two weeks is not a long time at all, especially when

you compare that time with the time it took for you to get to where you are now.

I'm going into week 7 of my first challenge and I've seen small changes. I'm

expecting to see many more positive changes as I keep going.

Many times I've read on this forum that there are those who don't see real

change until the 8 week mark.

Keep going. One day at a time. Pamela had a great suggestion. Share your

meals and maybe someone can help you see where you may be missing it.

What are you doing for cardio? Have you done all of your workouts in that 2

week period?

Also look in the files section for " Lady Success Journal, " or

something like that. THere are a lot of stories of seeing major

changes 8-9 wks into the challenge.

Changes are happening if you're following the program, even if you

can't see them.

- not sure if you got your questions answered. But you can fly

into any of the DC area airports - BWI, Dulles or National. There is a

shuttle to Children's Inn from each of the airports and it runs every

couple of hours. We flew into Dulles, but only because the flight was

direct and at a convenient time. I think driving distance is about the

same from all the airports....

Good luck!

There are three airports in the Washington DC Bethesda area.

Are you working with Rhonda??

She should be guiding you. Will depend on your home airport and the time you

need to fly.

Dulles is the one most fly into... but I am not one that likes that

airport... ONLY because out of Charlotte we arrive far from the terminal and

have to walk and walk and walk... LOL

As I remember the original post, this doctor has no staff, no nurse,

nothing. I suggested she ask for an alternate number (ya, I mean HOME number),

so

she can contact the doc when a problem occurs.

I still get the eeby-geebies just THINKING about this situation... your kid

on meds & no way to contact doc. Hmmmm... did I spell " eeby-geebies "

correctly?? :-)

Thank you SO MUCH for your help! I can't tell you how good it feels to have

a support system. I always feel like I am going through this alone and you

guys have been GREAT!

What dosage and what medications is your child on? Does your doctor start

the medication slowly or at full dose and if it is slowly at what point do you

increase it to meet the OCD problems?

Thanks

He tried Lexapro first - it worked so we haven't had to do any fancy

switches.

20mg is the usual top dose for ocd (though I have read lately that

lexapro doesn't actually have a certain top dose yet . . .which I

take to mean that higher is possible)

He started at 5mg - became fuzzy/sleepy, as though it was 2am and

he'd gotten up . . . all day long (kind of cute really).

This started to fade after about two weeks so I increased the dose to

10mg.

Same thing, 2 weeks of fuzzy/sleepy, started to fade and increased to

15 . . . and same thing until 20mg.

At 20mg the fuzzy/sleepy wore off and out popped out a happy regular

boy without the " impending sense of doom " , need to have a zillion

rules about germs and cleanliness.

OCD is still there - in the same general form - it just isn't as

intense or as " dangerdangerdanger!!! " as it was -

Full effect at 12 weeks is true in my opinion -

He even made a suggestion that maybe he doesn't need to take it

anymore because he doesn't seem to have ocd very bad anymore - but I

told him that's real easy to say when it's doing its job and the ocd

seems to not really be there anymore. He dropped the suggestion and

takes it most every day on his own.

Oh, another thing, he had about a 5 week stretch during the ramp up

from 5mg to 20 mg where he became ticcy - face scrunches and head

jerks and anger flareups. . . he'd never been ticcy so I found it

kind of scary and worrisome.

But, it faded away and has never returned.

I read somewhere (or his doctor told me) that this flareup in tics is

more common with kids with add or adhd - that it's nothing to worry

about - and that " wait and see " is the best plan

I lost the plot with my son

I'll bet this is an Aussie term! I'm not familiar with it. However, from

your post I'm gathering that you lost your temper and feel badly. I think we

can all relate to those times when we say something we regret, especially

when pushed to our last nerve. I still remember when my now 13 y.o. was 3 -- I

called a customer who was a therapist and asked, " Is it normal to hate your

3 year old " because I was so driven crazy by her! I think she was showing

signs of OCD even back then. I think the times we " lose it " can be learning

tools; they can learn that parents aren't perfect either, AND they can learn

what an apology sounds like. Good luck -- you are NOT a bad mom -- just a very

HUMAN one!! Suzanne in CA
