Guest guest Posted July 1, 1999 Report Share Posted July 1, 1999 Hi Helen, and all, This can mean different dosages depending on whether you have been on long term prednisone in the past and your past response to it, how serious the inflammation is, and other health factors, etc. A typical pred burst for an adult as far as my experience as a patient goes is starting at around 30 or 40 mg a day of oral prednisone, tapering down by 10 mg each time, with two days at each dosage. For example, I am on this dosage right now: 30 mg (divided into two 15mg doses per day) for two days, then 20mg for two days, 10 mg for two days, then to continue on 5mg thereafter. Normally I would stop after two days at 5 mg but I have not been able to keep the inflammation down in the past with nsaids when flaring,so we are trying a 5mg continuous dose for awhile to see if we can get my hands and feet out of this flare. I think that some manufacturers package it up in special dose-paks for this kind of treatment as I have heard others talk of them. I imagine you know this, but just to be on the safe side.....when taking prednisone, it is very important to be under a doctor's supervision concerning the dosage as stopping or dropping the dose too quickly can have serious effects. I'm not keen on using prednisone generally due to all the problems it can cause, but have had no luck controlling the inflammation recently by other means. I was beginning to get quite concerned about possible joint damage in a couple of joints and having difficulty walking or I would not be doing it myself right now. My personal two cents is--try all other alternatives first before considering this. I was able to keep the inflammation down to a reasonable level for some time just using herbal treatment and an occasional aspirin. I guess I'm in the two-stepper's-back club right now! My peak flows are usually in the 570's and have been in the 450's, so just into yellow for me. But it seems once I drop below 90%, I am on a hair trigger and it can drop to 50% quickly if I exert myself. I swam and ran competitive with undiagnosed asthma as a kid and teenager and that really stretched my lungs. Unfortunately I do have scarring in one lung. So my peak flows look good, but my air exchange is poor. Also my diaphragm is very strong so I can force the air in and out but I get tired and hurt. Thankfully my doctors are taking my symptoms into consideration and not just looking at my numbers. I have woken the past two mornings feeling rested and relaxed - not like I had wrestled with an elephant and lost. So the meds are helping. I am feeling better ashtma wise, but with just 3 days in I am starting all my usual prednisone side effects...which are quite nasty and individual to everyone! My family dr. also added in Theophylline 300mg daily just over 1 year ago and at the time, it really helped me and I continue to take it and have my blood tested regularly. Since being on the Xolair I have blown over 500 on a couple of occassions! WOW for me!!! I just hope my insurance company doesn't give me a hard time because I think that when I got my approval last year, they were going to reasses after 12 months...(maybe they will forget!!). I went from around 350 to around 450 with the usual suite of meds and now from 450 to 570 (hard to believe) and rising with Xolair.
