Call your insurance company and ask how to file a medically necessary form or a

grievance form. If they tell you that you must write a letter make sure you

include all of your symptoms, meds currently on, and that Xolair is supposed to

give you back function of your lungs, without it, they will go right back to

where they were.

If they say your physician must write the letter ask his nurse to do it and

include everything above as well as start date of xolair prednisone dose before

xolair, prednisone does now...well that goes for all of your meds, dose before

xolair & now. If they say they don't do the appeals, let me know, I'll writeit

for you. That is part of my job and I do it well. I was called a " formidable

witch " by a DR who was head of an ins. co. in California. He then apologized

and gave the person her Xolair for an indefinite period of time. That was my

greatest accomplishment.
