TRT is usually prescibed in cases of primary testicular failure, or

when other options (lifestyle changes, lowering E2, Clomid, hCG, GnRH)

fail. Whether or not you want to father children is also a factor.

Pros and Cons: Too numerous to detail here

Cost: Usually lower for hCG vs. TRT (I believe).

In reality I doubt that one endo in a thousand knows anything about HCG use

for hypo so TRT ends out getting prescribed for any case where the T is low.

You are right though that it should only be prescribed for prmary failure.

Yes and no. HCG will cost about $50.00 per month more or less depending on

dosage. TRT with a generic cream from one of the compounding pharmacies

will be about $20 per month but about $200 per month for Androgel. With

insurance copay Androgel is still more expensive than compounded even if you

have insurance (plus you have to fight with them for approval). It is

argued sometimes that androgel has a patented delivery system which makes it

absorb better but even if that is true you could use more compounded and

still come out ahead. I would think.

For costs analysis.

Remember, often a man on T injections or HCG will have elevated

estradiol levels, which means the need to add an anti aromatise in

order to truly benefit from treatment. At this time, anti aromatise

like Arimidex and Aromasin are quite expensive. So this will add to

the costs significantly if estradiol gets too high. That being said,

some men are getting good success using arimidex in very low doses

(like a pill a week), so that lowers the costs significantly in that

case. Some guys though, like Oreon, use at least 1/2 pill a day, and

that adds siginificant costs to treamtent. As far as I can tell, TRT

with Androgel or compounded cream causes less aromatisation and

estradiol on average than T injections or HCG. According to the men

on this board, Pellets seem to cause the least aromatisation,

however, I am not sure if that is a proven fact yet (not sure if any

studies have been done on this).

Copay cost for AndroGel is probably variable. I had BC/BS high risk....when

I was on 5 gm/Dy copay was $25.00. When I was on 10 gm/Dy...Copay was

$25.00. So it appeared to be Rx dependent.

I was on compounded T cream once and I found it to be very messy.
