Hi again all, I just got my test results back from my doc that I

requested.

Thanks to this group, I knew what questions to ask and what tests to

ask for.

My T levels have been between 117 and 190 for the last 4 months. I

have gone from AG to Testim and I feel better with the Testim (and my

balls are shrinking).

Here are the results I got back:

I am 34 Yr.

T 96 Lab range of 280 - 1300

Free T 26.8 Lab range of 47 - 244

LH .3 Lab range of 1.5 - 9.3

FSH .4 Lab range of 1.4 - 14.6

Prolactin 9.6 Lab range of 2.1 - 17.7

E2 21 Lab range of 18 - 67

I am glad to see that my Prolactin and E2 are ok, but what is going

on with my T.

I would also say that boy's are OK, and can prob produce T, but it

looks like they are not getting the signal to make any T.

Where to go from here?

Thanks, Nick

I think your assesment is right on. The fact you have any T what so

ever with such low LH and FSH is testiment to being able to at least

produce something. Have you ever had the Clomid stim test described

in the AACE guidelines? (see links or files section of the board).

That would tell you for sure.

First comment on your results is that by these you really can't see

one absolute cause. Assuming secondary (which it may be primary) I

would think you will need to have your cortisol numbers in hand.

Your E2 seems fine so that's likely not providing any negative

feedback to the hypothalimus but you did not show any E1 (estrone)

numbers and E1 can also provide negative feedback tot he

hypothalimus. Hagh levels of E1 can be derived from abnormally

highfuntioning adrenal producing high levels of DHEA. DHEA is

aromatized (via the aromatase enzyme) into E1. Outside of that, high

levels of cortisol, use of glucocortisoids (cortisone shots,

creams), or use of prohormones and/or steroids are all circumstances

where your HPTA can shut down. Your body's natural production of

cortisol may be out of wack so I would ask to have this tested if

you have not already. Exercise can induce high levels of cortisol

shutting down your HPTA.

Outside of any of these, your next step is to have a pititary MRI

done to verify the absense of any pituitary lessions. While your

prolactin looks fine, you could still have a non-cancerous growth

there that is interfering with your axis.

As far as next steps go, if it were me, I would seek a pituitary

MRI, testign of my cortisol levels, and a Clomid Stimulation test.

Good luck.

Thanks for the info, it is driving me crazy that my T levels are now

down to 97, it it normal for the T level to drop during TRT?

I will look into the Clomid stim test and the others you mentioned.

I do know that I am normally under large amounts of stress from work

and also had a new son 2 days ago.

I have an appt with an endo at the end of this month.

Thanks again.

Congratulations on the birth on your Son. Being there for our kids

and loved ones is another great motivator for all of us to get and

keep healthy.

Stress will certainly do it. I'm in sales, very stressful at times,

so that may have been a factor for me as I am sure it is for you.

Regarding if T levels can drop while on TRT, an article I just read

when makign my decision to go on the compounded T mentions that in

many cases where a patient still has moderate function/production of

T, TRT will often show a reduction in levels in the second and third

week after therapy begins. The article says that this is caused by

the body recognizing the new T and shutting down the current

production. In cases where the patient has only mild to moderate

hypogonadism, many times the dosages will be increased to

compensate. Specifically with topically applied testosterone.

Articles from my doctor's site are here:

Hope all of my wanderings makes sense.
