Not to add too much confusion to this subject but it was always my

understanding that (up to 100cm is Proximal) (100cm-150cm is medial) (150cm+

is

distal)

Did I get that wrong?

as far as ins. companies are concerned - at least the 10 or so companies I

routinely work with - 100 or less is proximal or short, over 100 is long or

distal and they want no part of it, will typically deny coverage for a long

limb bypass.
