My current assignment is to write a scholarly paper, and I really wanted to center it around celiac disease by conducting a survey having to do with the diagnosis of celiac disease. If you could fill out the survey, and also distribute it to your family members (or fill it out for them if they're young children), I would greatly appreciate it. My essay is not being published or anything, and if you'd like to read it I'll send a copy! Everything will be kept confidential and anonymous. In my paper I will basically be collecting data from the surveys I receive back and stating my own conclusions based on that data. If you feel uncomfortable, just refrain from filling out the survey and I understand completely. If you can go ahead and fill it out though, please do so and send back as soon as possible!
