Laurie-

THANK YOU SO MUCH! You have relieved my mind. I was told by Dr. H in NY

that to have mito you had to have an elevated lactic acid level. Well I've

never had mine to be elevated and Dr. Cohen has never made anything of it. I

was really panicked after I've had this mito Dx for nearly 2 yrs for another

specialist to tell me that.

Was a little uncertain for a lil bit. I feel alot better now though. I have

total trust in Dr. Cohen and his definitive Dx. My muscle biopsy showed the

depletion of mitochondria.

Were these blood levels or CSF levels?

Were these blood levels or CSF levels?

The ratio is felt to be the indicator that my docs look at, but I think it

is a high ratio, not low (but not sure). My lactate is always in the normal

range, although I have never had it drawn during a really bad crash.

I have a specific deficiency in my muscle biopsy, complex II/III confirming

mito and my lactate is normal. Pyruvate is high but I was told that a high

pyruvate with a normal lactate is useless..

Rest easy, an elevated lactic acid level is never elevated in some people. I

would trust Dr. C.

Hi ,

You are right, they do LOOK for a HIGH Lactate. But if your Lactate is LOW, that

doesn't mean that you DON'T have MITO.

Most times my Lactate is HIGH, Only once did it come back LOW.

Although my Pyruvate is ALWAYS LOW.!!

I honestly don't think they are looking at the lactate levels as closely or as

much as they did years ago. Most doctors seem to not comment on the results

unless it is totally out of the normal ranges.

So rest assured, take a deep breath, you have your Mito dx with your muscle

biopsy. Gee..that sounds like you should be happy that you have Mito, I didn't

mean it to sound that way, you know what I mean.

I had high blood lactate (from arm draw) once, out of about 7 tests.

My pyruvate is always low, the ratio of lactate to pyruvate ranged

from normal (10 - 20) up to 100 or more (indicative of mitochondrial

problems).

I have a " light case " of mitochondrial disease, symptom-wise, but

interestingly, I've had a high lactate/pyruvate ratio (over 100

sometimes) and zero Complex I activity upon fresh muscle biopsy (I

think normal for my age was 100 activity, and normal for older people

was 50 activity - I had ZERO), as well as lactic acid buildup in the

brain.

I think it's difficult to get a good lactate/pyruvate blood test

at " the local hospital " , as there are things like not putting on a

tourniquet and adding the perchloric acid right away to the sample

that most local places wouldn't know about. None of the local blood

drawing stations would do it, and my local hospital redid my first

set of tests four times before they got it right. Now I call them to

make sure they have the materials and staff ready before I go in.

I also have taken the prescription for the lab tests to the lab the

day before, so they can figure out how many of what kind tubes etc.

I haven't heard of high pyruvate in mitochondrial disease, but

remember most of us have different " flavors " of mitochondrial

disease, even if the same complex is affected, so don't let our

experiences affect your discussions with your doctor. My doctor had

a problem where the nurse had reviewed my results and said they

were " okay " because the lactate was normal and the pyruvate was low -

usually I guess " low " is considered " good " . When the doctor reviewed

my results, she knew they supported the mito disorder diagnosis, and

sent me for further tests.
