Pain medications for late onset metabolic myopathies

I need some suggestions for medicine to help my severe, dull, chronic pain. Right now I'm on 20 mg oxycontin 2X. But after being on for over a year and percocet for 10 years, it's not helping me as much as before as my condition worsens (chronic pain, exercise intolerance, extreme fatigue and tiredness, no energy, sleepiness). Now my doctors are looking at fentanyl patches or methadone. Also have diabetic peripheral neuropathy which has not been alleviated with standard medications (neurotonin,etc). And the usual stuff for my terrible muscle stiffness even makes the pain worse. I would appreciate any info on your experiences with pain killers for these symptoms.
