I would suggest having her seen by a GI for reflux. Almost all of these problems can be explained with this. All three of my girls had severe reflux at a young age. They even become rigid and pulled back, because it just hurts. I also have reflux issues, and I notice myself pulling back, for some reason it kinda helps the pain. I nursed all of my girls for as long as possible, and had the same problems with the gagging ect. Unfortunately I was unable to nurse any of them due to problems, (6) I got pregnant with Bre-Anne(5) and dried up, with Bre-Anne she had severe reflux and had to be put on Progestimil for the reflux, and with Grace (my mito kid, 19months) she had the same issues as Bre but just worse. Grace used a formula called Neocate. Both Progestimil and Neocate are already predigested and are very easy for the child to absorb, helping with the reflux and sensitivity to the milk. The only difference is Progestimil is soy based and Neocate is amino acid based. The cost is astronomical, so make sure your dr. suggests them before you try them. Also all my kids used Pepcid. Grace eventually needed stronger and was switched to Prevacid. The medicine makes a huge difference. Good luck and hopefully all of this will just pass with the wind.
