Guest guest Posted February 4, 2004 Report Share Posted February 4, 2004 The State of California has denied my request for disability benefits---all because my doctor's office does not know how to correctly fill out the DE2501 form " Doctor's Certificate " . (quick recap-i had upper/lower surgery on Dec 12, 2003. I was off work for exactly 6 weeks, so I am basically missing out on 5 weeks of benefits, and i need this money!) There is no reason they should deny me, but the idiots at my doctors office dont know how to fill out the form, so i have taken it upon myself to do it. I have 20 days to appeal the denial, so I need to act quick. If anyone dealt with these California state forms, please let me know. I need to get the ICD9 primary & secondary disease code. one of the girls in the office thinks the codes are 2114.7 and 211.96. But how do i find out for sure?
