In SC its the same form from the health department.

Not sure about your state, however, it really should all be the same.

I did, and the Mom's day out used the same shot rules as the county school

system they are in. Don't now about your state, but is probably similar.

Does anyone know if the same vaccine waiver we sign for school

attendance is also the same for daycare? My daughter(NT)but only 14

mo. old is not recieving vaccinations. yet I would like to send her

to mothers day out one day a week, but they require shot records,

and I wanted of any of you have expereinced this. Thanks for your

It depends on the daycare. If it accepts any federal or state funds

[and most do, even church-based daycares], then they are required to

accept your state's vaccine-waiver requirements. If it is an entirely

private daycare, then it can make its own rules.

I believe that since the daycares are private vs. public that they will be

able to deny your child entrance based on your childs vaccine record.
