I think I was really at the limit, 11 or 12 cm is much bigger and the amount of time taken to cut it into little bits for removal would really be too long and I doubt whether even a very experienced surgeon would want to take it on. >can anyone please give me any feedback on their >experiences with Dr. West(NY), Glassner(Philly) or >Dr. (Calif). Guest guest Posted February 10, 2004 Report Share Posted February 10, 2004 There are a number of factors that we take into consideration when we consider laparoscopic myomectomy. Size and number of fibroids, position and depth of the fibroids within the uterine wall and desire for fertility are all considerations. It is actually easier to remove a 10 cm subserosal myoma laparoscopically than a 5 cm submucosal myoma because there is less uterine muscle to go through to get to the fibroid and, therefore, less bleeding. More fibroids take longer to remove and are more technically challenging than just a few fibroids. While any number of fibroids can be removed abdominally, I usually limit laparoscopic myomectomy to 5 or 6 fibroids. Repairing the uterine wall laparoscopically is a bit more technically difficult than with abdominal surgery. If a woman is interested in fertility I may recommend abdominal surgery if the fibroids are numerous or deep into the wall. I can then repair the uterine wall in a way that should withstand the forces associated with carrying a pregnancy for nine months. If fertility is not an issue, then there is much less concern about this and laparoscopic myomectomy can be safely considered. The " cutting up " part (morcellation) is actually easy now with the new instruments we have available, so this is rarely a limiting factor. I hope this is of some help.
