Guest guest Posted March 11, 2004 Report Share Posted March 11, 2004 I also filled out the FMLA paperwork. I did not have enough time to cover the 5 weeks I was out. I initially was going to be out 4 weeks, that was on my FMLA paperwork. My doctor faxed in a letter the 3rd week saying that due to complications, I would not be returning until after 5 weeks. This correspondence from my doctor was sufficient. I think a lot of people end up being gone not as long as or longer than the date initially put on the FMLA forms. That's difficult to determine before any surgical procedure, you can only estimate. I'm sure it is just a formality to fill out the forms, probably best to do so " just in case " as mentioned.
