Could one of you please, please look this up for us to try to find out if all of this is LEGAL? Although we all hope everything goes very well with the surgery, I personally would want to know if the phrase is satisfactory and legal and I surely would want to calm 's worries and fears! Can anyone out there help us please? AND if they DO allow me to write the above on the forms, does this mean i should feel more secure, and that I should go ahead and sign the consent form ?
