CONSENT FORM - worried, need advice

Dear Group Members:

Further to 's concerns below, I imagine there MUST be at least one attorney

here amongst us - or perhaps even a paralegal? Could one of you please, please

look this up for us to try to find out if all of this is LEGAL? Although we all

hope everything goes very well with the surgery, I personally would want to know

if the phrase is satisfactory and legal and I surely would want to calm 's

worries and fears! Can anyone out there help us please?

I am still not clear on this, and my MYO. is coming in Feb.

**Is it enough to write the following statement on the hospital admissions

form, as well as the informed consent documents before surgery ?

" I DO NOT WANT A HYSTERECTOMY UNDER ANY CIRCUMSTANCES OTHER THAN THOSE

NECESSARY TO SAVE MY LIFE " .

AND if they DO allow me to write the above on the forms, does this mean i

should feel more secure, and that I should go ahead and sign the consent form ?

My doc. may tell me he can't do the surgery unless I sign the consent form.
