As for wearing a cast three weeks after the tenotomy, Dr. Ponseti told me he padds the cast a lot to allow for growth; to keep it on a minimum of two weeks but really aim for the three if at all possible. You'll see signs if it becomes too tight, watch for swelling in the toes, blue or purple or reddening of the toes, the baby's obvious discomfort, etc. at which time you take them off yourself if necessary! The condition of the toes are your window so keep an eye on them closely. s. Anterior tibialis transfer Hi, I wonder if someone could give me some information on the anterior tibialis transfer surgery. My son is being treated with the Ponseti method, and has made good progress so far. He has had 5 casts so far. Next visit he is having the tenotomy, and then will be into the brace beginning September. However, his feet were quite severe and rigid to begin with. The Dr. told me that he may need this anterior tibialis transfer around 3 years old, and mentioned that about one third of patients need this. Now that surprised me, as I was of the impression that 95% of patients were corrected without surgery. I also thought that this surgery was usually due to non compliance with the bracing, and we haven't even started with the brace. Another question.... If the cast is left on for 3 weeks after the tenotomy, doesn't it get very tight on the baby's legs? is growing at such a rate, that I've been glad to have the casts changed weekly. The cast he is in now will be on for 2 weeks, as the Dr. is away next week. After that the next cast will be on for 3 weeks, but I worry that he will get very uncomfortable.
