Guest guest Posted September 2, 2004 Report Share Posted September 2, 2004 Hi Everyone, Im from the UK, and have recently found out during my 20wks scan that my baby has unilateral talipes equinovarus(on right foot). The babies left foot looks fine. I have been given no information regarding Talipes and dont know anyone with this condition. So pretty clueless! Anyway done a little researching, found out that the ortho doc I should see is useless! Have been told to stay clear of this doctor. However have been recomended a doctor in Sunderland who does Ponseti method and worked in America with Dr Ponseti. This is not to far away from where I live so going to ask to see him. However when does treatment start after birth? I will be having a C- Section at a different hospital. Also baby is due last week of November, so not to far away from Christmas. If it has weekly casts what happens at Christmas and New Year? Will the depart still do a weekly cast? Worried Docs will be on holiday and disrupt treatment. I am from Beverley, near Hull, and have a 6 month old boy called Ben with unilateral (left)clubfoot. Mr G V is our consultant (Hull Royal Infirmary) and he and his team of physiotherapists are Ponseti trained and are excellent. I have heard of the consultant in Sunderland (Mr DeKiewet?) and he is said to be excellent also. Another option is Naomi of Manchester-you can actually e-mail her with general questions and she always answers! Another good source of info about the treatment is Dr Ponseti's website (you should be able to find it on google or something similar-let me know if any problems). In answer to your query, the treatment usually starts 7-10 days after the baby is born-weekly plaster casts for 6-7 weeks. This is usually followed by a tenotomy (tiny incision in the achilles tendon to stretch it), then back in plaster for a full 3 weeks. The foot is then fully corrected but this needs to be maintained with boots and bar (often referred to as DBB) which are worn 23 hours a day for 3 months and then night time and naps only until age 4 or thereabouts. The first week in the boots was a nightmare but no problem thereafter. Check out the Markell website (makers of the boots). We also found out about Ben's talipes at the 20 week scan. It was upsetting at first but the more you find out about it the easier it gets. They like to start the casting treatment on the baby within the first 10 days after birth. Casts are changed every 5-7 days. If you're still going through casting during the holidays, they'll work the cast change schedule to accomodate the baby. (they *should* at least!) I'm sure you can discuss this with the doctor ahead of time to find out when he'll be out of the office and who covers for him while he's gone. The doctor should be willing to meet with you ahead of time (in person or on the phone) to discuss the treatment and answer your questions. It sounds like you have done a good bit of research already and are on the right track. There are a couple of people in the group from England and I am sure they will chime in. As far as casting goes, I am sure you will be able to work around the hols. If you already know who you will be seeing you can plan it so that the casts are changed a little earlier or a little later than a week apart and make all your appts at one time. We made about 6 appts all at one time after Zoe's first cast. Some parents who have traveled miles and miles with their babies to Iowa City to see Dr. P have their casts changed every 5 days to speed the process, so they can get back home. I will let someone else comment on what is the longest is. The cast after the tenotomy stays on for 3 weeks, so I am sure a few extra days over the week period will not be a problem.
