I always take what I call my dissertation to every doctor I see. On it, it has my current medications, the doses, who prescribed them and why, my allergies, the list of my doctors, their specialites and phone numbers as well as all surgeries as well as the reason for my visit and any additional back ground information. After the last couple of days I came across a GREAT pain notebook to keep if you are seeing anyone, pain specialist or not for your pain. It gives a very detailed version of what one's daily pain is really like hour by hour and the things you do for it. This can be found at http://painfoundation.org/ Look toward the middle to end of the page, I believe it is the second paragraph and it gives you a place within the paragraph to print out an Adobe Acrobat form of the log book. It is only necessary to print out pages 8 and 9, not all 24!!! The instructions are the 4 pages before that. I do hope this helps others as I am sure it will help me.
