right wording for consent form ?

Is this enough to write on the hospital admissions form, AND the informed

consent documents before surgery ? Thanks to Suzanne, she just sent me this

wording from Carla's book. Thanks Suzanne and Carla !

" I do not want a hysterectomy under ANY circumstances other than those

necessary to save my life " .
