Hi.My daughter just turned four and she is due three shots.I am trying to

figure out how to get an exemption in the state of Kentucky.Her doctor is

pushing

for her shots.Thanks,ne

Thanks Dana.This may sound stupid but how do I do a written sworn statement

in Kentucky based on religion.Do I have to specify a religion? Do I have to go

to court?Do I go to a notary?Will this be OK for her private school?She goes

part time.Will hte health department accept this if they go to her

school?Thanks alot!!!ne

The code says this:

2) the 'parents are opposed to medical immunization against disease,

and who object by a written sworn statement to the immunization of

such child on religious grounds.'

So you write something like this:

" I am the parent of [child's name], a minor child. I object to

medical immunization against disease on religious grounds. Therefore

I will not agree to immunizing my child. "

I don't know the requirements in Kentucky for the " sworn " statement.

If the school accepts any federal or state funds, and most do even tho

they are private, then they are required to accept your state's

exemption. Bring a copy of the code with you, just in case. If you

are challenged, say " here is the law. My statement conforms with the

law. Where do you find that more is required? "
