Sanna,

It sounds like the doctor is going thru the early Ponseti jitters and desire to

modify. I would not change the angle unless Dr P advises you to. Keep it at

70 and let the doctor see that the 70 degrees does not hurt his foot, that will

help him learn that there is no need to modify it ;)

Smaller angle in FAB?

Hi all,

Good news first: Veeti's original doctor has finally trained in Iowa

City with Dr. Ponseti, and is now using the Ponseti method in

clubfoot treatment here in Oulu.

We had a leg&foot check up with him, and he wanted to decrease the

abduction angle in Veeti's FAB by at least 10 degrees. Is this

normal? Dr. Ponseti told us that we should use the brace 14

hours/day until he is three years old, and has said nothing about

decreasing the angle. The foot looks great and is flexible and shows

no sings of relapse, but the doctor is worried that Veeti's foot

will develop flat if we keep using it in 70-degree-abduction.

Veeti's left leg is still 1,5 cm shorter than the right one, and we

have to go back in three months to take x-rays from his back and

both legs. We also have an appointment to a genetist to find out if

Veeti has some kind of syndrome that causes all the left side's

anomalies (syndactyly, clubfoot, leg lenght difference, poorer

hearing in left ear..).

That was our news in short. I'm happy to see so many new families on

this board and sorry that haven't got the time to participate more.

And many thanks to Charity who patiently waited for Veeti's foot

prints for the quilt. I can't believe it takes more a week for an

express letter to arrive from Finland to US!!

Is he in the straight last or the reverse last shoes? My son was in the

reverse last for a while and the doctor had that set at 45 degrees because

of the fact that the reverse last shoes already bend the foot out. Once he

was able to wear the straight last shoes, the angle was changed to 70

degrees. If he is in the straight last shoes, I'd leave it at 70 degrees.

If he is in the reverse last shoes, as far as I know, they should be at a 45

degree angle. Also, our doctor told us that all babies who have clubfoot

will have a flat foot, so if this is true, changing the angle isn't going to

make him not have a flat foot.

Hi Sanna-

First off congrats on urging your first doc to train with Dr. P. We got our

first doc to finally train with him too.

My son is 2 and a unilateral left. We've never had to change the

degrees on his clubfoot, but have had to several times on his normal foot.

Sometimes he walks with his normal foot turned out more than his clubfoot

and so then we alter the degrees until he walks normal again (usually only

takes a couple of days), in the past he has also intoed too much with his

normal foot, so we have had to adjust it back out. It is just something we

watch regularly. His clubfoot has stayed about the same. I had mentioned

this on the board when it started happening and there were several parents

who said they've had to adjust the clubfoot or normal foot (shoe) for

different cases. I think best to email Dr. P himself and tell him what doc

has told you to do and get his input.

I totally agree with what the others have said about the angle. My son

Jakob wore his FAB set at 70 degrees the whole time he wore it (3 1/2

years). As for the " flat feet " theory, I don't think the angle of the

FAB should affect it. Jakob's feet are not flat at all. I think we

were told at one point (not by an ortho) that the arches don't fully

develop until a child is at least 2 years old.
