Tonia,

I have heart problems and had heart surgery in 2000, and my doc just raised my methadone to 60mg every 12 hours. It is the third time in the last four months that he has raised it and told me that you need to raise it slowly to find the amount that really helps. We are going to try and stick at this level, but he knows about my heart and blood pressure and is not worried at all. He is also very good and one of the best docs I have ever seen, and came VERY HIGHLY recomended! I think it is amazing that we are all getting such different information, but I am sticking to what I know right now, because this drug has helped me more than anything else I have taken! Wow I did not know this, I will have to check this out with my doctor. I am on 30mg 3x a day.

I went and seen my pain doctor on Thursday....and asked him to raise the dose on my Methadone From 10 mg every 6 hours. He told me that if he raised it up from that, that it causes SEVERE HEART PROBLEMS at higher doses. So far, my pain doctor has done everything possible to help me, so I know he wouldn't lie to me.

Just wanted to warn everyone who's on it.

Re the discussion on Methadone..............................

All of this is pretty confusing to me. Just this past Thursday, my doc switched me from DIlaudid to Methadone. The reason? Despite heavy duty efforts, we could not get get the Dialudid increased to more than 8mg four times a day without my being majorly dead and drowsy (even with his having added in Ritalin to counteract the drowsiness and difficulties with remaining alert AND with adding in the Meclazine to counter the balance issues that came from a vestibular side effect to the Dialudid).

He discontinued the Dilaudid and, while keeping both the Meclazine and the Ritalin in place, started me on the Methadone at what he described as a very small beginning dose (10mg at 8AM, 5mg at noon and another 5mg at 4PM) and he was extremely clear that he would be steadily increasing it. He stated that it would be necessary for him to carefully pace the increases due to how apparently sensitive I seem to be to narcotics, but there's no doubt in my mind that he plans to keep increasing it over the weeks to come.

With his knowing that I have a lot hinging on my being able to come up with some sort of medication plan that will simultaneously adequately address my pain AND eventually permit me to return to work, he described a number of "if this doesn't work then we will try this....and if that doesn't do it then we will go with.....all the way forward to a plan that may include his prescribing medication that will necessitate my having to give myself injections at various points throughout the day. Given my discussion with him (and I have absolutely no doubt in his expertise in the area of pain management...he is both an oncologist and has board certification in pain management), I am positive that he views Methadone as an acceptable and appropriate pharmacological treatment for the management of severe intractable chronic pain. I realize that methadone is also used as a substitute for heroin withdrawal, but it is a narcotic analgesic.

I don't know if what I am adding here is helpful or not, but just thought I'd share my own (albeit short) experience. I see my doc again tomorrow and will let you all know what he does with my next increase. I would have written sooner, but as some of you know, I flew down to Baltimore with a friend Thursday evening and just returned this afternoon.... and that's "material" for another post. Missed you all and kinda apologize for having to delete some of the 378 emails to which I returned (not even included "bulk" mail) !!! I'll try to catch up tomorrow.
