reg. Tenotomy

Dear Friends,

My son has a unilateral club foot. He is 7 months old. I started

his treatment recently. My Doctor is saying that, he wants to do

tenotomy after 2nd cast and then he will continue casting for 4 more

casts(Total 6 casts). This is because, he said that the soul is very

stiff. SO by above method only he can do proper correction.

What is your advice? As far as I know tenotomy is done before last

cast and the boy should remain in cats for 3 weeks after which Shoe

is given. How do we approach this? If tenotomy is performed after 2nd

cast, should we wait for 3 weeks to continue rremaining casts or

should we continue with weekly casts? Your advice is needed.

Apart from this, the boy is given Local Anaeshtesia using Kitamine.

Are there any side effects of this drug? What is the normal procedure

of anaesthesia?

I am sure someone in here will be able to help you out more than I could.

My doctor is casting my 6 ½ month old son 3 times before his tenotomy and

the last 3 week cast. He will be putting my son to sleep because (as you

know) they can really move. I would just want to know why your doctor is

doing it under local? Dr. Herzenberg said that they would use local on the

younger babies who aren’t as strong and mobile as they are at 6 months and

up.

Hi there,

It sounds like you are aware of how the Ponseti Method works and that

what your doctor is proposing does not follow the method. Your son

should have the series of casts done before the tenotomy and the 3

week cast after the tenotomy is so the tendon can heal in the

position it will be in SHOES, not more casts. He shouldn't need more

manipulation after the surgery. Red flags!!

Can you consider checking with another doctor?

I would encourage you to email Dr. Ponseti directly. You can reach

him via email at: ignacio-ponseti@ uiowa.edu.

Just explain what your doctor is suggesting just as you have done

here. He will be able to advise you better whether or not proper

correction can or cannot be achieved through these means.

I would definitely get a second opinion fast. Where do you live so

maybe someone on the board can suggest someone who they have

experience with.

Dear All,

Thanks very much for all your response. The main reason for Local

Anaesthesia was my son developed edema in Lungs in Larynx when he was

treated for a surgery. Hence doctor said they will go for Ketamine or

Gas Mask.

Secondly, I am not clear about the 3 casts and then tenotomy and then

3 more casts. I live in India. I am getting him treated in Gujarat.

So, please let me know.

Hi,

To clear it up, the tenotomy is done last. After the procedure (I hate

calling it a surgery - it's misinterpreted too much as the big surgery)

baby gets a cast which is left on for 3w. After that they go into the

DBB/FAB. Have you looked through the information on Dr. Ponseti's website

about the casting process? It is typically 5-7 (sometimes less) plaster

casts, each one does a different thing and looks slightly different as far

as how the foot is positioned, then the tenotomy and the last cast stays on

for 3w while it heals in the over corrected position (this is so the tendon

regenerates longer than before).

Perhaps the Dr. is just planning on changing the tenotomy cast weekly? I

think you should clarify this with the Dr. and ask if there will be further

correction in each cast after the tenotomy, or if he's just changing casts

to check the incision (which is unnecessary).

Check out the website with the pictures of each cast. It shows the

difference between them as well as describing the correct process with the

tenotomy at the end of the correction. Do you need the address for the

website?
