I had to have my address and Dr phone numbers changed. My Walgreens has a pad of forms by the pharmacy checkout. They send it off to some company. Guest guest Posted May 9, 2004 Report Share Posted May 9, 2004 , I ended up getting a medic alert necklace instead of the bracelet, as I could fit more info on that and I have trouble with things on my wrist. The necklace itself was free, but I have to pay a fee every year to continue on with the Medic Alert services. I can update them with new diagnosis or with changes in medications at any time, along with contact information. I hope you find something that works for you.
