The procedures I have done are all performed in an outpatient pain clinic here, and not a hospital (with the exception of the SCS trial). Unfortunately, it's not entirely surprising to me that the lidocaine wasn't very helpful for him. My constitution is such that strange things work for me (as all of the nurses had told me that it was rare for them to see an RSD patient maintain good relief from lidocaine infusions) while other things that help others, or appear to be "breakthrough" treatments (like ketamine) are in fact, not my friend. Frustrating I know, when no one treatment, or even list of symptoms matches another. Hang in there and hopefully the doctors will find something that will help him find a more comfortable level. What state are you from & which hospital administered the lidocaine for you? He has not noticed any improvement in any area of his body.
