Hi Carolyn,

Sorry to hear things are no better there seizure wise :(

How are the med reductions going? Maybe you might be going a bit too fast

with them if he is not improving? I have fallen way behind on messages, but

catching up now, so not sure if I have missed any in between.

I have had a look at ael's height/weight on the growth charts, but

just wanted to check I had it right..

I get his weight as just under the 20th percentile, and his height as up

at the 90th - is he exactly 6 yrs old? When was his 6th birthday?

If exactly 6, his BMI (body mass index, or height/weight ratio) is very

low at 12.24, which according to my charts, is actually even below the 5th

percentile?? In fact not even on the chart! When you say 1.21 m, do you mean

121 cm?

And yes, his diet calculations should ultimately be aimed towards more his

ideal weight, but if you are already struggling with ketones getting up

higher, increasing calories right now may make this even harder to achieve.

Have all his medications/vitamins etc carb content been taken into

account?

At his existing weight, cals would be set between 1170 and 1224 cals per

day, slowly inching up to get some weight gain over time. (As Freeman's book

recommends, if the child is underweight, start at existing weight, them work

upwards - otherwise he will be on too many cals right now to be mimicing

starvation until he 'grows into' a higher cal allowance)

With a lot of seizures, so probably little normal activity, he may be

actually better off at the lower end of the scale, ie - the 1170 mark. This

sounds very low for a 6 yr old though, so maybe somewhere in between?

Protein is usually 1.1 per kg, again, usually set to ideal weight, but

that would be too high for ael right now, so on his current weight,

19.8 gms, or your 20 gms, sounds about right.

So on my mealplanner with 1224 cals and 20 gms prt on a 4:1 ratio, I get

122.4 fat, and 10.6 carb.

Are you checking glucose levels? As if all meds have been calculated in, I

cannot see at the moment why his blood ketones are looking so low. What are

his urine ketones showing as?

Keep a close eye on his weight over the next week or two, as this may give

you a clue if those cals are indeed a bit on the high side for his current

metabolic/cal burning state.

That's about all I can think of for now, hope today has been slightly

better for him,
