Guest guest Posted July 28, 2004 Report Share Posted July 28, 2004 Hi , Just saw a post come in from you while I am plodding thru my mail, and wanted to jump in ask your opinion on 's latest blood gases...our hosp is keen on him starting bicarb, but because his ph is at an ok level, I am thinking that looking at the other results, he seems to be doing an ok job himself keeping things stable? The compensatory alklaline swings seem to have taken a hike for now, so do you agree I should leave well enough alone? Still seizure free as far as I can tell. Value your acidity/alakalinity knowledge as always Ph : 7.38 (um, can't remember reference range..) bicarb : 17 (24-29) base excess -7 (-2 - +2) blood gas - 3.8 (4.6 - 6.0) - they termed it blood gas over the phone, but it goes by another name on the lab print-out that I can't remember) Thanks Quote Link to comment Share on other sites More sharing options...

First I thought CO2 but the reference ranges for that are much higher.....gee, thats gonna bug me now! Anyway I would like to know what it is before saying for sure, but I would say the other ones look good to me. If he is stable seizurewize and not showing any outward signs of any problems, I sure as heck wouldn't mess and add bicarb. Like I said I would like to know what the last one just to feel completely confident though. Guest guest Posted July 28, 2004 Report Share Posted July 28, 2004 K, just received the 'hard copy' and it says on there 'Gases pC02 lood' - 3.8 (ref range 4.6 - 6.0). So mebbe a diff measuring value to the one used over there or something? I would have thought his CO2 would have been raised if he is acidic, not lowered? Or would that only be if he was in respiratory acidosis? Or am I getting this all mixed up again Ugh. These were all venous bloods btw, not arterial, didn't want to put him through that, but I think it is accurate enough to use as another baseline. The question about 's recent lack of growth again has been raised as being possibly linked to either his chronic acidity from the diet/topomax combo, or that his cals and/or protein may still need tweaking up after his catabolic period last yr, or due to a low blood level of zinc - which can apparently affect growth as well as hair, skin, teeth etc. I'll carry on with his current med/diet regime I think, poss lower the ratio down a bit from 3.5:1 to 3.3:1, and wait for him to have a follow up ree dx test to reassess cals, dietician is planning on booking one for him in the next few weeks. I'm looking into a decent zinc supplement too, so hopefully that should all cover my bases, except for the bicarb to reduce the acidity of course, which I am not going to use at this point....(the co2 reading shouldn't be changed my opinion should it?) Kidney ultrasound next week should hopefully tell us if any nasties forming there, which is the other topomax worry nagging away at me. Sooooo - how is it not weighing anything? Actually there are two different measurements CO2 and pCO2. Its been explained to me what the pCO2 is but I can't remember, anyway its not the same as the CO2 measurement. However CO2 will actually be lower if you are acidic because........sheesh, see, even though I " know " this stuff, I still really don't, anyway it has to do with the heavy breathing (which by the way I NEVER see with , despite how acidic she gets), anyway they manage to breathe off the CO2 as a compensatory mechanism to help lower the acidosis, or something like that > Or would that only be if he was in > respiratory acidosis? Nope. It will be interesting (real interesting) to see if Jess starts to grow now that she's off the diet. God, I hope she does! We see neuro in a couple of weeks and he wants to do another kidney ultrasound now that she is off the diet. Thought that was strange cuz we just did one in Feb but whatever.......I asked for a 24 hr urine to make sure she is no longer peeing out all her calcium and nurse clinician wanted to do keto bloodwork now that she is off to compare to their baseline prediet. So looks like we are back for the full meal deal this time, everything except bone densoromitry (which we did in March), despite fact neuro only wanted ultrasound LOL > Sooooo - how is it not weighing anything? As well as the mixer, that thing has sat in a place of honour on my cupboard beside the scale for over six years. I couldn't even figure out where to put the blender to put it away LOL Still only have a partially happy girl though. Sometimes she is so excited and happy cuz she is eating the same as everyone else, other times all she focuses on is the few things she can't have. Kids.........
