(Feel free to email me the form off-list.) I've got access to several police rider forms, but am looking for a fire/EMS specific one due to HIPPA and other health-care specific issues. The January 2006 edition of JEMS had several points relating to making a ride-along policy " legal " in light of HIPPA and I'm going to try to incorporate some of those points.
