Frequent Spitting up, despite Nissen

My son Ben (5 month old CHARGEr, 2 months early) recently left

Childrens after a long 4 months. He has a trach, G-tube and Nissen. For

the first three weeks after the G-tube/Nissen surgery, he didn't retch

or spit up. On December 23, he started retching and spitting up minimal

amounts in the morning - every morning. The last four days, it has

gotten a lot worse. He is doing it throughout the day and the retching

is getting harder. Ben has a lot of mucus and secretions (he also has

coanal artresia that keeps on closing up after surgery). We are trying

everything the doctors have recommended (changed his formula to Enfamil

AR, started nebulizer treatments 4 times a day, lowered his calories,

propped his bed, leaving him vented to the farrell all day) and nothing

seems to help. Ben is still on continuous feeds too. We have gotten a

chest xray, a head ultrasound and had his Nissen checked to make sure

it was still in place - and everything seems fine.

Has anyone else had similar problems? Any suggestions?

Hello Amy,

I am sure you will get some wonderful advice from a lot of moms on this

listserv who have experienced the woes of the nissen. But I have to ask you,

since we both go to Children's Hosptial in Cincinnati, who was your sons G-tube

surgeon? If it was Dr. Inge, he probably told you that he doesn't need reflux

medicines because of the nissen. This is not neccesarrily true, there are alot

of kids who still have to have reflux meds in spite of the nissen.

Also alot of times tests do not reveal what all the symptoms are pointing

Telling you something is wrong. Also is he gagging and retching forcefully? He

may not be refluxing, so much as he is trying to vomit. I can only tell you my

story, there are so many others who have theres to tell. had a hiatal

hernia that caused her to retch and vomit, she could not tolerate her feeds,

even at minimal amounts, the stomach could not decide what to do with the food,

wether it was going to go up or down, had an upper G.I. done which

showed that her nissen was intact, ten days later, she had another upper G.I.

which showed that her stomach was up inside her chest cavity(poor baby) and

revealed that she had the hernia. The surgeon was in absolute shock, he said

that if her name had not been on the test, that he would have never guessed in a

million years that it was hers, because it was so abnormal from the other upper

G.I. that had only been done 10 days before.

My guess is to call the Aerodigestive clinic, I hope you have Maureen

Gallagher as your feeding coordinator, what a marvelous person she is! Don't

know what we would have ever done without her. She is our angel at Children's. I

would definitely ask for her if you can and I know that she will work doubly

hard to try to help you figure things out

for you and Ben. I hope that the situation with the spitting up and nissen is

resolved soon.
