All of that has to be filled out and faxed back. You will also be required to have labs and an ultra sound of your gall bladder (if you still have one). All depends on how fast you can get your paperwork in. I faxed mine the next day (I do not have a gall bladder)and my PCP worked with me and ordered labs the very next day which were faxed down to PB in two days. A week later PB called and scheduled the consultation for the end of the month. You fly down for one day--see three different doctors--then fly back. After about two weeks you will get your date, which ranges from one to two months. I had my consultation on July 30th and got my date on September 18, 2002.
