By definition, Small Duct PSC is diagnosed

by symptoms and histologic findings that are consistent with PSC, after

elimination of other possible causes…in small duct PSC the strictured

ducts cannot be visualized or are extremely difficult to visualize

radiographically. So although you’re correct that biopsy may not be

conclusive, and despite the fact that ERCP is considered the gold standard for

definitive diagnosis of PSC, I don’t find it unusual that the ERCP was

negative.

I do agree with your comfort level

regarding being followed by a GP. I wasn’t comfortable being followed by

anyone other than a highly skilled Hepatologist.
