Guest guest
I know this sounds dumb, but do you need a letter from your doctor in defense that is medically necessary? I have the copy of the script that she wrote originally. I also have a page letter from my PT (which he has sinced graduated from) and a letter from the cranial facial specialist. Do you think that is enough? I too, am trying to get this appeal thing going. It is sooo overwhelming, the whole project. I know many of you have so much experience with this, is there a list of what would help to be enclosed in our appeals packet? For the medically necessary defense? Thank you, JOanne and Cameron, 9 months PHX, AZ Rec'd 8-24-04 DOC band > > From: Filyaw <monicash@...> > Date: 2004/10/20 Wed PM 04:31:20 EDT > Plagiocephaly > Subject: Re: letter of med. necc. > > , > Our first letter was one paragraph. In all honesty our dr did not realize the insurance fight for coverage of this. His usual one paragraph usually saw great results. So, it may be your ped just didn't realize. When it was time for a second band he got a sample letter from CT and he actually wrote a whole page. > > > mom to na > > cartersdoc <mamagill@...> wrote: > > Hi all. Guest guest
In order to pre-authorize we had to submit a letter of medical necessary... which ended up being a paragraph that states: " has bilaterally flattened occiput symmetrical; because of recommendations for back to sleep it was difficult to utilize repositioning. He has a symmetrical problem therefore repositioning would not be effective alone. Now that he is 6 months old he can be up and off of his back more, the DOC Band would actively improve his head shape. " That's all she wrote... : ( > > > > Hi all. Short and to the point - i.e. repo is not going to work he needs a helmet! cartersdoc <mamagill@...> wrote: In order to pre-authorize we had to submit a letter of medical necessary... which ended up being a paragraph that states:" has bilaterally flattened occiput symmetrical; because of recommendations for back to sleep it was difficult to utilize repositioning. Guest guest
Thanks... I'm still weary of it but i guess you're right. I just kinda hoped she'd at least write a bit more. >From: Filyaw <monicash@...> >Reply-Plagiocephaly >Plagiocephaly >Subject: Re:( letter of med. necc. >Date: Fri, 22 Oct 2004 12:03:35 -0700 (PDT) > >Actually that's a very good letter.
