Contest Information

Requirements:

Babies in treatment Nov 2003 – Sept 2004

Color prints (film preferred)*

Information on back of photo:

(use label or soft pencil)

- Baby's name

- Clinic location

- Parents' name/address

* Due to calendar printing requirements, we can use digital format

only at a resolution of 4 megapixels or higher (300 dpi at 5 x 8),

which many personal-use cameras do not have. Digital color prints

must be included along with a CD or disc.

Deadline: September 17, 2004

Is anybody doing this? Just curious. I don't think I have that many

cute pics of him in his band. But it would be really cool to see him

in it next year.

My daughter isn't able to be featured in the 2005 calender, but I

did it last year and she was featured as one of the " Miss January's

2004 " . I have cut her out of the one I have in my office and I tape

her to the current month. hehe, proud momma!

We did this last year and Hannah was chosen as Miss July 2004!

She's the chub on the beach . She's actually eligible again this

year, lol. I would encourage all DOCparents to do this, b/c even

though they get a ton of entries, it is such a neat keepsake to have

if your baby is chosen!
