Progress notes

We combine our progress note with a request for recert so I would say yes. At

the top of the form are boxes that indicate that the form fills multiple

functions : 10 vitis/monthly summary/ recertification/ dicharge.

There is a place for physician signatures on the same form.

under a POC that is for 90 days. A progress note is completed at 30 days

and 60 days per Medicare requirement of progress note 10 visits or 30 days,

whichever is first. Now you have a POC due at 90 days if the patient is

continuing therapy. Does the updated POC take the place of the progress

note at 90 days or are both required at 90 days?

Because we have several FIs for the clinics in our company & not all have

embraced a 90 day cert period, we have chosen to continue with a 30 day

cert period for all clinics. Regardless, in the situation you describe I

believe that the 90 day cert takes the place of the progress note as long

as your visits match up (i.e. if you have seen the pt for more than 10

visits between the previous progress note & day 90, you would have to do a

progress note at the 10th visit & a cert at 90 days). This is no

different than if you were doing a 30 day cert period & had seen the

patient for 10 visits within that timeframe. The only time you need to do

a progress note sooner than 30 days is when your visit frequency is 3x/wk

or more. Hope this helps....

I'm confused, with MC in OP. a progress note need be written how often if

you are seeing he pt 2x/wk. and I mean progress note in chart not progress

note to MD for re cert. could a weekly note be done? or is daily (as we do)

still the way to go?
