Guest guest Posted September 19, 2006 Report Share Posted September 19, 2006 I need to complete the therapy cap exemption form for a spinal cord injury patient of mine (she met her cap at another facility earlier this year). This is my first time to complete the form, and I am wondering about the 'Number of Exception Days Requested' section. Let's say I request 2x per week for 6 weeks (12 visits). If she continues to show progress after those 12 visits and would benefit from more PT, will it be difficult to be approved for more? If anyone has any additional suggestions I would appreciate input. Thanks in advance.
