Per CMS:

The surrogate UPINs are as follows

Active military doctors and those employed by Veterans Affairs: VAD000

Doctors serving in Public Health System, including Indian Affairs: PHS000

Doctors who do not have a UPIN, such as pediatricians, some mental health

providers, or for diagnostic or radiology doctors: OTH000
