Guest guest Posted April 12, 2001 Report Share Posted April 12, 2001 Hi I'm new to this group having found it about one week ago. I'm 35 and have had fibroids for the past 10 years. A few years ago I had a laproscopy & D & C to remove fibroids from within my uterus. This helped immensely to relieve heavy bleeding. Unfortunately I missed my annual check up with my gyn last year. I am now in the position of having fibroids the size of a 31 week preganancy. These affect my bladder, bowels etc. My local gyn (I live in country Australia) said I must have a mymectomy, but says due to the size of them, there is a high chance of hysterectomy. He has also decided not to operate, suggesting I have the procedure done in the city - more experienced gyns. I have not had children and in the last few weeks have had to contemplate the possibility of not having any. This is devastating. Anyhow, I'd be interested in anyone who has had fibroids the size of mine - and what treatment options they've tried. I am currently on monthly injections of zoladex (had first injection 3 weeks ago - no symptoms thus far). My gyn says I should be on this for 3 months, and then I must have the surgery. (ie shouldn't wait). Next week I will travel to the city to meet the doctor my local gyn has referred me to, and schedule the surgery. Back pain, etc. I'm seeing my gp tomorrow and will ask him to take more tests. Have a call in to my gyn which will be returned tomorrow morning. Will then discuss the laparoscopy with him. Did anyone have a laparoscopy in the initial stages and what was it like. Besides two childbirths, I have not ever had an operation and am so scared...this stuff is more depressing than I expected. Guest guest Posted April 18, 2001 Report Share Posted April 18, 2001 Hi Connie I've been a member of the group for a couple of months now and have found it a great source of info, support etc - I wish I had advice on experience for you but I am in wait and watch mode ( getting close to menopause - I hope! - and not going to have more children), while I learn enough to feel I can go ahead with a particular procedure. The reason I am replying to you is that you are the first person I have seen who is from Australia - I am in the country here in NSW and would have to go to Sydney for any good treatment - I would just like to know if it is Sydney you will be going to and if you would have any recommendations. It would be great to be able to get to the States for treatment but financially prohibitive!! Any way good luck and I am sure you will find a way other than hysterectomy - I know you have had some good advice already from someone here on the site. Please let us know how things go for you. I've had the urine tests at many different times and the presence of blood has nothing to do with my period. he sent me to another specialist who couldn't find anything even after using a cathether test. I visit my gyn about every 6 months. Lately, I seem to be there every month! Thanks for your response. I will continue to try to find an answer to this problem as I think it's related to the fact that I now have ovarian cysts as well as fibroids. I'd had a year of progressively heavier bleeding and severe cramping. I had an endomitrial biopsy that didn't really tell us much. As the bleeding began to get worse and the cramping would turn me white as a sheet and have me doubled over in pain we started to get more serious about finding out what was wrong. When my period lasted for 40 days non-stop in January of 1991 my doctor finally said we had to do the laparoscopy and find out what was going on. What he discovered was that I had 4-5 golf ball sized fibroids and endomitriosis. I had a myomectomy in March of 1991 to remove the fibroids and had the endomitriosis removed by laser. What a difference. I went from having periods that were extremely painful and which pain medication barely muted with bleeding that would go through a super tampon and super pad in 10 minutes to having periods that had absolutely no pain and lasted 2-3 days at most. I was in heaven with the changes. I was scared too I hate being put out and feeling out of control. I had some pain because of the gas they inject to distend the abdomen like a tent. I also had my right ovary removed because of endometriosis and they did it through another very small incision. All in all it wasn't bad. I was up and about in a few days. The Dr. said because it is submucosal it is probably the one causing the excessive bleeding. I didn't have laporoscopy for fibroids, it was for endometriosis so I am sorry I can't comment on how they would accomplish that. I am sure someone else can. Good luck. Guest guest Posted April 20, 2001 Report Share Posted April 20, 2001 Hi Freda During a regular internal examination, my GP felt something unusual (distoted cervix or odd placed ovaries, I think) and referred me to a gyn who did an ultrasound, and discovered the fibroids. Initially I was told to take a watch and wait approach, with annual ultrasounds to check on them. I did this, until the fibroids in teh uterus grew to such a size that my periods were very heaby and I was iron deficient. I then had a laproscopy & D & C and my gyn removed the fibroids from within the uterus. I was a little anxious but this procedure was relatively pain free (except I happended to get a UTI the day before the procedure - Ouch!) In any case I have had normal periods since then but the fibroids out of the uterus have grown very large - not helped by the fact that I missed one of my annual gyn check - ups. Now I beleive I have very large fibroids, most of which are out of the uterus. I have just returned from the specialist in the city ( I live in the country) and will be having a mymectomy in July. He warned me that die to the size of mine, there is up to a 50% chance of a hysterectomy. This makes me concerned. However, I feel I should still go ahead with the procedure. I have not ever had blood in my urine, sorry I can't identify with that. She has a myomectomy planned for July, but has been told there is a 50% chance of it developing into a hysterectomy. Connie, I don't think these are acceptable odds. Any responsible doctor should tell you that a myomectomy may be converted to hysterectomy due to heavy bleeding. The overall % I think is only about 1 per cent, but bear in mind that doctors will be selective and mostly operate on fibroids which they think are amenable to myomectomy. But I wonder whether use of a GnRH agonist (Lupron, Zoladex, Synerel, etc) to shrink the fibroids beforehand, and/or going to a surgeon more competent in myomectomy might be advisable in your more complicated case. When myomectomy is thought to be that risky, I would definitely investigate UAE. Sometimes subserosal fibroids (growing on the outside surface of the womb walls) don't reduce sufficiently after UAE. At least, if the UAE doesn't give good results, you still have the option of surgery. Rarely, hysterectomy is needed for severe infections. If the problem is simply continuing discomfort due to bulk of fibroids, myomectomy after embo should not entail much blood loss. Chances are you could have UAE with no need for further treatment. If you want to explore this option, you could look at the SCVIR website www.SCVIR.org and look up the general info on Uterine Artery Embolization. Once you have basic info on the procedure, if you are in N America you can look up an Interventional Radiologist to consult with http://www.SCVIR.org/locate/index.htm If you are in Australia, there are some women here or at the embo list (another Yahoo group) who could tell you whom they consulted. There are or were even contacts in Japan and Pakistan at the Yahoo embo group. If in the UK, some radiologists are listed at the Database list of consultants at http://www.smartgroups.com/groups/fibroids And you can ask your GP for a referral to the nearest available Interventional Radiologist, or contact hospitals and ask them if they have an Interventional Radiology Dept.
