Dear Derrick

Well I am sorry of course to hear that you have reached this point with

your IGAN, but it sounds as though you were expecting it. Nevertheless,

this is not a nice thing to have happen, however much it looked as

though it was on the cards. Know that I wish you lots of luck with the

surgery whenever it takes place, and know also that you will have the

support of the group along your journey. I hope that once you start

dialysis you start to feel much better.

Pierre and group,

As I predicted, I saw my neph this afternoon, he

looked at my lab results and said " I think it is time to do a

fistula, I would like it done before Christmas if possible " so there

we are, I have finally made it. Strange thing, he did not ask if I

would like HD or PD, guess he assumed HD. Having seen your

experience, Pierre, I think I can handle that. I see the vein Surgeon

next week to make arrangements.

Sorry to hear you're soon going another step closer, but I really do think

it's a good thing to be having the fistula surgery soon. Because it takes so

long for a fistula to mature, I felt it was reassuring after I had it, and I

just hoped it would be long enough until dialysis was needed so that the

fistula would be good and ready (which is how it turned out - I started hemo

6 months after the fistula surgery).

I don't think I ever once heard my pre-esrd nephrologist mention PD

herself - only hemo and transplant. I only heard it from other members of

the renal team. The social worker was really the only one who seemed to

encourage PD. Maybe in your case, as in mine, there was an " assumed " reason

for hemo ultimately being the choice (I mean a medical reason), but one that

was never really stated to you. I get the feeling that you would have chosen

hemo anyway, but if not, you could always ask. It should be your decision.

Some nephrologists do have their own preference, and they do exert a lot of

influence on patients' ultimate choice, I think. Keep in mind that if there

are no medical contraindications, the choice of hemo vs PD is not written in

stone. Many would say it's not a bad idea to have a working fistula

regardless - there is if it's needed, sort of thing.

it with Pierre, I can't wait to get it over with now. I see the

conceited Surgeon for assessment or what ever they do next Tuesday

prior to making arrangements for surgery.

I am as prepared mentally as I can be thanks to you and the

group. I just can't wait to get it finalised and over and done with

now which is a far cry from where I was 9 months ago! I see the

conceited (Frances's joke) Surgeon next Tuesday arvo to make

arrangements. The sooner the better.

That's how it was done for me, Derrick. An appointment with the vascular

surgeon first, so that he/she can evaluate your veins and decide where to

put it... and then the actual surgery. In my case, that came about 3 weeks

after the consultation.

P.S. These decisions apparently aren't too firm, because once I was on the

operating table, the surgeon decided to make the vein-artery connection at

the elbow instead of at the wrist.

Derrick,

That pretty much sums up how I felt at the time. I was glad to have it under

my belt. There really isn't much to it you know.

If my experience helped you in any way, all I can say is that I'm honoured

that I could be of assistance to such a fine gentleman as yourself.

Vascular surgeon? I would like to commend the person who used it because I like

vein surgeon better. It is more descriptive. :-)

Well, technically, they deal with both arteries and veins, so a " vein

surgeon " would only be able to do half the job : )

Derrick, You've received such positive encouragement and support, as you

have given to others in the past. We will be thinking about you as you

travel through this new venue and into your new life style. Good luck with

Yes, the anticipation is almost exciting, but not quite. I

would like to get it over before Christmas if I can, I suppose it

depends on how busy the surgeon is. The strange this is Connie, I do

not feel bad at all, I am ticking along OK at present.

You cannot escape the inevitable, I have been preparing myself

for this for quite some time now, I think it would almost be an

anticlimax if it didn't happen.

I saw the vein surgeon today. As you

predicted, Pierre, not much to it. He examined my arms, decided which

vein to use and signed me up for an op on 17 December. That took 15

mins and cost $120. Thanks for your prayers, , good to have re

inforcement at a time like this. Thanks for your concern too, Jennie,

being in the medical profession you would have an idea of what is

involved although Melbourne is another world away!

Thanks, Derrick. Well, yes, I must say that we've had a few patients on my ward

who have had fistulas put in. They've all come through it just fine, so I hope

it gives you a bit of reassurance. All the best for the 17th. Let us know how it

goes.

Hi Derrick,

My apologies for the delay in writing, I have just caught up on my

emails.

It's not nice spending so much money on specialists, and I bet the gap

is nearly 50%.

And you pay the full fee just because, you could have been in there for

5 minutes for all you know, and still paid the full initial fee.

I have entered it into my calendar to remember your surgery for Dec 17,

it will be fine, no need for luck. Enjoy the deep sleep, that is if you

get knocked out completely for it?

Hey everyone, talking about paying, do you pay everytime you see your

Neph?

I have heard some people see their Neph on Medicare. I pay $85AUD each

time, and get back less than half.

Yes it is about the same for me, $85 per visit with about 50%

rebate from Medicare. You are between a rock and a hard place, I look

at it from the point of view that you need the best so you have to

pay.

When I went for that consultation, I was first seen by a pretty vascular

nurse who said I had nice veins, I was very happy about that : )

$120 eh? Better start saving your pennies for the post-surgery follow-up

appointment : )

I've never had to pay a single penny here, for anything. Well, it's 4 PM

here, so, I'm off to dialysis.

Frances,

I knew you wouldn't be able to resist that one. Talking about

the weather, it is snowing in the Snowy Mountains south of Sydney,

the temperature in Sydney has dropped, but that doesn't help the

bushfires, they are raging because of the high winds. I think the

whole world has gone mad 'cept thee and me and I have mi doubts about

thee.

You sure are a wit! In the South East of England where I live we have snow

forecast for this weekend, but judging by the sky, it looks like it could be

here today. Strange, because yesterday I didn't wear a coat down the beach

whilst walking the dog, yet today its freezing. Perhaps the world has in

fact gone schizophrenic! Incidentally I hear the set of Home and Away copped

it in the fire. Well, they say some good always comes out of bad!
