Please answer the following questions if you will: 1. Is pain scale used and documented at every single visit? 2. If it is not used at every single visit, does your facility have a policy to address when it may not be used (ie, if there is no pain ID'd at eval...or pain documentation by exception if nothing ID'd at eval).
