The original when to the medical record and the cc went to billing. For examples email me directly. Now, I am working in the ARU setting and our therapists document and insert charges into MediTech. My health information manager is saying that we cannot keep charge tickets in a patient record, since this reflects " the patient's ability to pay " . My argument is that the charge ticket is reflection of services provided during a therapy treatment, independent of the patient's payor source. Guest guest Posted September 30, 2004 Report Share Posted September 30, 2004 , This past April we began using a form that serves as both our charge sheet and daily note. The charge sheet is on the left hand of the page and our daily note on the right. This form was approved by our HIM department as well as an outside consultant. You're right-- It does not reflect on the individual's ability to pay, only what was billed. The form has worked well and provides excellent Quality Assurance data. I am currently using it to study whether PT/PTA documentation of services actually reflects what was billed. This is easy in that all the information I need is maintained in the patient chart on this one form. I have been able to identify whether the staff actually has a clear understanding of how to bill units and Rx time, group versus direct exercise, appropriate use of 59 modifier, etc. We are very pleased with how the dual form is working. Guest guest Posted October 3, 2004 Report Share Posted October 3, 2004 Jim, We send the carbon copy of our note/charge sheet to data processing where the charge is entered by a clerk. The clerk then sends the charge sheets back to our department along with a computer print-out of the patients billed and their charges. Our secretary double checks the print-out against the charge sheets to verify that they have been correctly billed.
