I am puzzled by how you explain about the information. Don't take this the wrong way, my difficulty comes in not understanding how the contact was made. I can empathize with you when it comes to getting forms filled out. Are there any advocate resources that you could turn to for help? The way medicine is going today many doctors have heavy caseloads and many don't receive compensation for filling out forms. Other than this I don't have much to offer, sorry. Be Blessed D.
